Comparative StudyThe contribution of the obturator nerve block in the transurethral resection of bladder tumors.
This study was designed to evaluate the efficacy of local block of the obturator nerve to avoid it's stimulation during transurethral resection (TUR) or tumors localised on the lateral bladder wall. Forty-five patients were studied, in whom the TUR operations were performed under spinal anaesthesia. ⋯ In the first group no stimulation was observed while in 5 patients of the second group was not possible to totally resect the tumor due to the persistent stimulation of the obturator nerve. In conclusion the local block of the obturator nerve is an effective method to avoid it's stimulation, easily performed and without complications.

Five cases of acute allergic reaction to latex are reported. Four of those were diagnosed during surgery and presented anaphylactic shock. ⋯ Increasing frequency of latex allergy is reported in the literature and is usually seen in patients with extensive neurosurgical or urological histories, or by peoples currently exposed to natural rubber products (medical staff). Systematic screening in history before surgery is important so that exposure to latex should be avoided in suspected cases.

NSAID are by far the most efficient therapy in renal colic treatment as they directly interfere with the physiopathology of a renal colic. Other therapies that have been standard are now shown to be insufficient or even contraindicated. Hydratation during a renal colic seems to be unimportant. The efficiency of stone-expulsive drugs has still to be proven.

[Patient-controlled epidural analgesia after major urologic surgery].
Postoperative pain is an important issue after major urological surgery. Efficient analgesia is mandatory. The administration of i.m. narcotics is considered routine. ⋯ This pump also allows patients to administer themselves additional boluses according to their needs. Sixty-two urological patients used this system to their own satisfaction and the satisfaction of the nursing and medical staff. This new approach of postoperative pain relief is discussed.

[Percutaneous nephrostomy and emergency drainage in obstructive anuria].
Sonography seems in patients with acute renal failure a reliable method for diagnosing urinary tract obstruction, giving the kidney size and localizing the kidney for percutaneous drainage. Percutaneous nephrostomy is actually the best temporary derivation for patients with obstructive anuria before definitive treatment. 18 cases of obstructive anuria are reviewed. Urinary lithiasis is the main aetiology (13/18). 11 of our patient had undergone percutaneous nephrostomy as a first step of their treatment.
