Pro's and con's of different blocks for postoperative analgesia after video-assisted thoracic surgery.
Although there are a lot of studies examining the effects of different blocks for postoperative analgesia after video-assisted thoracic surgery (VATS), the results are controversial. ⋯ All three blocks can be suggested for daily practice, the choice should be based on personal experience and preference of the anesthetist. Better studies are required for objective decision.

The role of extracorporeal membrane oxygenation in thoracic anesthesia.
Circulatory and respiratory support with extracorporeal membrane oxygenation (ECMO) has gained widespread acceptance during high-end thoracic surgery. The purpose of this review is to summarize the recent knowledge and give an outlook for future developments. ⋯ Clinical practice during surgery on ECMO is increasingly personalized. The effect of personalization on morbidity and mortality must be examined in the future. Undoubtedly, an increase in knowledge can be expected from this trend towards personalization.

Dystonia is common in Parkinson disease patients, affecting about 30% of them. Bilateral subthalamic nucleus deep brain stimulation (DBS) can sometimes lead to dystonia, but this relationship is not well understood. Our aim was to provide a better understanding of dystonia's causes and its connection to DBS. ⋯ Patients with off-dystonia before surgery tend to improve after DBS. Otherwise, DBS could have the role of "additive boost" in the process of sensitization of striato-pallidal pathways and lead to on-dystonia in particular patients.

The letter responds to a study on the role of erector spinae muscle quality in surgical decision-making for lumbar spondylolisthesis. It highlights the potential of paraspinal muscle fatty infiltration as a predictive factor, suggesting that early rehabilitation targeting these muscles may reduce the need for surgery. The authors propose improvements for future research, such as using advanced MRI techniques for better fat infiltration assessment, controlling patient activity levels, and incorporating multidimensional imaging analyses with machine learning. These advancements could support more personalized treatment strategies for lumbar spondylolisthesis.
