By targeting the medial branches of the dorsal rami, radiofrequency ablation and facet joint injections can provide temporary amelioration of facet joint-producing (or facetogenic) back pain. The authors used CyberKnife radiosurgery to denervate affected facet joints with the goal of obtaining a less invasive yet more thorough and durable antinociceptive rhizotomy. ⋯ These preliminary results suggest that CyberKnife radiosurgery could be a safe, effective, and non-invasive alternative to radiofrequency ablation for managing facetogenic back pain. No patient suffered recurrent symptoms after radiosurgery. It is not yet known whether pain relief due to such lesions will be more durable than that produced by alternative procedures. A larger series of patients with long-term follow-up is ongoing.

Review[Neurostimulation in patients with chronic neuropathic pain in the so-called failed back surgery syndrome (world and the author's experience)].
The failed back surgery syndrome (FBSS) is defined as protracted or recurrent pain, mainly in the lower back and/or legs, even after previous anatomically successful spinal surgery. Treatment of such patients commonly involves difficulties since neither medical therapy nor repeated back surgery does not result in adequate pain relief. Spinal cord stimulation (SCS) is a minimally invasive procedure that allows physicians and patients to define the effect of therapy just before permanent implantation. ⋯ Many patients told about fair therapeutic effects and the occurrence of mild side effects. Thus, SCS is an accessible alternative to traditional therapy. It is the treatment of choice in drug-resistant FBSS patients where recurrent neuropathic pin persists after surgery and analgesics are no longer effective.

Due to the exactly placed injections pain can be overcome. ⋯ The complex injection technique affords knowledge and expertise. The exact application and the specific complications are presented in detail.

Kyphoplasty is a minimally invasive procedure that is increasingly used to treat pain caused by compression fractures of vertebral bodies. A 56-year-old woman who had a compression fracture on the vertebral body of L5 vertebra was admitted to the Algology Department with a severe low back and leg pain. ⋯ After injection of triamcinolone and bupivacaine transforaminally into the L5-S1 anterior epidural space, her pain complaints ended. If radicular pain symptoms caused by cement leakage are secondary to a chemically mediated non-cellular inflammatory reaction, transforaminal epidural steroid injection should be useful.

To describe how patients presenting with inflammatory back pain (IBP) of short duration can be classified by different sets of classification criteria for spondyloarthritis (SpA) and ankylosing spondylitis, and which clinical and imaging features are of discernible importance. ⋯ Among our selected cohort of patients with early IBP, the prevalence of SpA according to three different criteria sets is high. The ESSG criteria were the most sensitive, followed by the Amor criteria and the Berlin criteria. The modified New York criteria for ankylosing spondylitis appeared to be the most specific. In this cohort, the contribution of MRI and human leucocyte antigen B27 to purely clinical criteria in making a diagnosis of axial SpA was limited.
