Spinal endoscopy is a useful tool for the management of intractable low back or radicular pain originating from post lumbar laminectomy syndrome, epidural scarring, or disc protrusions, and non-responsive to conservative modalities and other interventional techniques including fluoroscopically directed epidural steroid injections and percutaneous adhesiolysis. Spinal endoscopy requires that the caudal canal be entered via the sacral hiatus. ⋯ In such cases, the procedure is stopped because of the absence of an alternative approach to enter the epidural space with the spinal endoscope, resulting in non-availability of this treatment. This report describes a novel method of dealing with the problem of cartilaginous obstruction of the sacral hiatus, using a mini-surgical approach to decompress the hiatus, allowing access into the caudal canal.

Ever since its initial development in the late 1960s, spinal cord stimulation (SCS) has been used to treat a number of painful conditions. European practice, in contrast to that in North America, has used peripheral arterial disease (PAD) as a primary indication for SCS. First employed in patients with PAD in 1976, SCS was shown by Cook et al. to heal chronic leg ulcers. ⋯ Recent randomized prospective studies have questioned some of the conclusions from these preceding retrospective data. In addition to the questions related to outcomes, theories regarding exact mechanisms by which SCS improves circulatory parameters remain unclear. A thorough Medline literature review on the subject of SCS in peripheral vascular disease was thus undertaken to attempt to clarify questions regarding which patients are best suited for SCS therapy, pinpoint possible methodologic flaws in previous studies, and to review the background, outcomes, mechanisms of action, complications, and alternatives for SCS in patients with PAD.

Distraction from chronic pain during a pain-inducing activity is associated with greater post-activity pain.
The aim of this study was to investigate the effects of distraction from pain during and after a pain-inducing lifting task in a sample of chronic low back pain (CLBP) patients. Fifty-two CLBP patients (25 males, 27 females; mean age=46.30 years) performed a pain-inducing lifting task twice, once alone and once with a simultaneous cognitive distraction task. ⋯ Further investigation of the catastrophizing data showed that the effect of catastrophizing about pain during the lifting task on the cognitive distraction task was mediated by the amount of attention paid to pain. Clinical implications of these findings are discussed.
