Salvage reconstruction in acute and late sequelae from pyogenic thoracolumbar infection.
Nine patients treated surgically for complicated pyogenic osteomyelitis of the thoracolumbar spine are reported. All patients were treated with anterior debridement and stabilization with nonvascularized autogenous fibular strut grafts. In addition, seven underwent a posterior spinal fusion with instrumentation. ⋯ Seven patients had postoperative improvement or resolution of their back pain. The use of nonvascularized autogenous fibular strut grafts for reconstruction of the spine following debridement for vertebral osteomyelitis is an effective procedure. The advantages of using the fibula for grafting are its strength and length for spanning several vertebral segments, and it can provide multiple grafts.

To investigate the factors associated with long term backache after childbirth, to assess all women reporting new onset long term backache, and to investigate any relation with pain relief in labour. ⋯ Though new long term backache is reported more commonly after epidural analgesia in labour, it tends to be postural and not severe. There were no differences in the nature of the backache between those who had or had not received epidural analgesia in labour.

Review[Problems in the treatment of back pain].
Back pain is a common disease causing tremendous costs for treatment, rehabilitation, litigation and work-loss. The reasons for back pain vary considerably and are often not clear. The effectiveness of current treatment concepts like epidural injection of steroids, blocks of facet joints, transcutaneous electrical nerve stimulation and antidepressants has not been proven. In accordance with literature, only multimodal concepts of treatment seem to be more successful as they consider somatic, psychosocial, and sports physiological aspects.
