Normalization of activities in daily living is an important goal in rehabilitation treatment of chronic lower back pain (CLBP) patients. Clinicians indicate that CLBP patients often show deconditioning but also CLBP patients who seem to be too active are seen. The objective of the present cross-sectional study was to gain more insight into the daily activity pattern of CLBP patients compared to controls, using accelerometry. ⋯ Overall activity levels do not differ significantly between CLBP patients and controls, but the distribution of activities over the day differs significantly.

Randomized Controlled Trial Comparative StudyEfficacy and treatment compliance of a home-based rehabilitation programme for chronic low back pain: a randomized, controlled study.
To assess the efficacy and treatment compliance of a home-based rehabilitation programme for chronic low back pain (CLBP). ⋯ Our results suggest that a home-based rehabilitation programme is as effective as standard physical therapy. However, this type of programme requires patient motivation and regular follow-up.

Ultrasound guided, painful electrical stimulation of lumbar facet joint structures: an experimental model of acute low back pain.
Quantitative sensory testing has indicated generalized muscle hyperalgesia in patients with chronic low back pain. The temporal development of such hyperalgesia is not well understood. The aim of the present study was to demonstrate whether generalized muscle hyperalgesia can develop within minutes of acute low back pain using a new experimental model of lumbar facet joint pain. ⋯ Electrical facet joint stimulation induced low back pain and pain referral into the anterior leg, ipsilaterally, proximal to the knee, similar to what is observed clinically. Pressure pain thresholds did not change significantly before, during and after facet joint stimulation. In conclusion, we describe a novel model of acute experimental low back pain and demonstrate that generalized hyperalgesia did not develop within minutes of acute low back pain.

The prevalence of lumbar spinal stenosis (LSS) in the general population and association with low back pain (LBP) remain unclear. ⋯ The prevalence of congenital and acquired LSS in a community-based sample was characterized. The prevalence of acquired stenosis increased with age. LSS is associated with a threefold higher risk of experiencing LBP.

The number of patients suffering from degenerative diseases in the lumbar spine is increasing in Japan. Although various scales to measure disability or quality of life in patients with low back pain and/or lumbar diseases are currently available, it has been shown that one questionnaire is not always compatible with another. Our purpose is to evaluate the association and differences between the Japanese version of the Roland-Morris Disability Questionnaire and the Japanese Orthopaedic Association score for low back pain. ⋯ The clinical signs in patients with low back pain and/or lumbar diseases are not associated closely with the Japanese version of the Roland-Morris Disability Questionnaire. Therefore, a combination of the Japanese version of the Roland-Morris Disability Questionnaire and the Japanese Orthopaedic Association score can provide wide-ranging assessment of the level of impairment in patients with low back pain and/or lumbar diseases.
