Lumbar radicular pain is a fairly common health problem, yet its risk factors are far from clear. There are no published systematic reviews on associations between cardiovascular or lifestyle risk factors and lumbar radicular pain or sciatica. The aim of this systematic literature review was to assess associations between these risk factors and lumbar radicular pain or sciatica. ⋯ No consistent associations were found for serum lipids levels or high blood pressure. In summary, the associations of overweight, long smoking history, high physical activity and a high serum C-reactive protein level with lumbar radicular pain or sciatica were substantiated by the present review. However, more prospective studies are needed in order to further clarify these associations and the mechanisms of action.

Correlates of self-reported disability in patients with low back pain: the role of fear-avoidance beliefs.
The purpose of the study was to examine the determinants of self-reported disability - MODQ (Modified Oswestry Low Back Pain Disability Questionnaire) in patients with low back pain (LBP); and to examine the level of FABQ (Fear-Avoidance Belief Questionnaires) scores in an ambulatory clinical population referred to physiotherapy management. We believe that identifying potentially modifiable determinants of disability in patients with LBP provides an opportunity to broaden the strategies to reduce its socioeconomic burden. ⋯ In this correlational study of physical impairments, psychosocial factors and self-reported disability, disability was driven primarily by measures of pain and fear-avoidance beliefs. Because the FABQ is a simple and useful clinical tool, we suggest that physicians and physiotherapists alike should make it a routine attempt to characterise the fear-avoidance beliefs of patients with LBP.

Effectiveness of an extension-oriented treatment approach in a subgroup of subjects with low back pain: a randomized clinical trial.
The purpose of this multicenter randomized clinical trial was to examine the effectiveness of an extension-oriented treatment approach (EOTA) in a subgroup of subjects with low back pain (LBP) who were hypothesized to benefit from the treatment compared with similar subjects who received a lumbar spine strengthening exercise program. ⋯ An EOTA was more effective than trunk strengthening exercise in a subgroup of subjects hypothesized to benefit from this treatment approach. Additional research is needed to explore whether an EOTA may benefit other subgroups of patients.

Effectiveness of a home exercise programme in low back pain: a randomized five-year follow-up study.
Therapeutic exercise has been shown to be beneficial in decreasing pain and in increasing functioning in patients with chronic low back pain. However, longitudinal follow-up studies are small in number, and often limited in the numbers of subjects due to drop-outs. In addition there is a shortage of real control groups in most cases. The purpose of the present study was to describe long-term changes in intensity of low back pain and in functioning for two study groups five years after undertaking a home exercise programme. ⋯ The present randomized study indicates that supervised, controlled home exercises lead to reduced low back pain, and that positive effects were preserved over five years.
