To evaluate muscle strength, as a component of physical deconditioning, and central activation ratio, representing the performance level during testing, in patients with chronic low back pain as compared to healthy controls, and to evaluate the contribution of cognitive-behavioral and pain-related factors to the central activation ration of patients with chronic low back pain. ⋯ When interpreting decreased muscle strength in terms of physical deconditioning in patients with chronic low back pain, submaximal performance has to be taken into account. The results suggest that patients with chronic low back pain who report increased psychologic distress and a higher level of current pain tend to show increased inhibition of muscle activity, leading to submaximal performance.

Long-term effect of a combined exercise and motivational program on the level of disability of patients with chronic low back pain.
A prospective clinical randomized controlled trial. ⋯ Regarding long-term efficacy, the combined exercise and motivation program was superior to the standard exercise program. Five years after the supervised combined exercise and motivational program, patients had significant improvements in disability, pain intensity, and working ability.

Intradiscal electrothermal annuloplasty (IDET) is a minimally invasive procedure for managing chronic discogenic low back pain (LBP). Although there have been numerous reports of IDET outcome rates, few have dissected the detailed factors affecting those outcomes. ⋯ Higher temperatures and larger total heating doses during IDET procedures with catheters placed in the outer annulus may increase the duration of post-procedure pain flare-ups and lead to less favorable outcomes at 8 months follow-up. The long-term outcomes at 16 months may, however, not be affected by these heating variables.

Clinical Trial Controlled Clinical TrialThe influence of different unloading positions upon stature recovery and paraspinal muscle activity.
To determine whether stature recovery and paraspinal muscle activity can be altered in individuals with and without chronic low-back pain by assuming different unloading positions. ⋯ Elevated muscle activity was found in the chronic low-back pain group supporting the existence of this explanation for delayed stature recovery. The gravity inverted position resulted in the lowest EMG and the greatest stature recovery. Further research is required to determine whether improving stature recovery has clinical implications by reducing pain/disability.

Controlled Clinical TrialSupplemental care with medication-assisted manipulation versus spinal manipulation therapy alone for patients with chronic low back pain.
To measure changes in pain and disability for chronic low-back pain patients receiving treatment with medication-assisted manipulation (MAM) and to compare these to changes in a group only receiving spinal manipulation. ⋯ Medication-assisted manipulation appears to offer some patients increased improvement in low back pain and disability. Further investigation of these apparent benefits in a randomized clinical trial is warranted.
