Chronic pain patients have complex problems. Due to this, much research effort has been expended on the classification of pain patients and the classification of pain problems. A mainstay of most pain classification systems is the use of the physical location of the pain. ⋯ Further, large demographic differences between patients with different painful sites, even when they have at least one pain site in common, suggests that grouping patients based on a single site descriptor may be inappropriate. Nonacetylated salicylates are frequently used in the treatment of musculoskeletal complaints and pain management in older patients because of their possible lower gastrointestinal and renal toxicity as compared with other nonsteroidal anti-inflammatory agents. We report five patients with chronic salicylate intoxication seen at the Atlanta VA Medical Center. All charts of patients diagnosed with salicylate toxicity were reviewed, and cases with acute, intentional intoxication were excluded. ⋯ A significantly decreased urate level and a wide anion gap were consistent findings as well. Two of the patients improved after hydration and discontinuation of use of the drug, two required alkalinization of the urine, and one required dialysis. Clinicians need to be more aware of the potential toxicity, including life-threatening complications, with the use of salicylates, particularly in high risk populations such as elderly patients and patients with multiple medical problems.

Positive results of pilot studies of the effect of staphylococcus toxoid vaccine in patients with fibromyalgia and chronic fatigue syndrome were the incitement to the present, placebo-controlled study. It included 28 patients who fulfilled the criteria for both fibromyalgia and chronic fatigue syndrome. The effect of vaccination with a staphylococcus toxoid was compared with the effect of injections of sterile water. ⋯ Fifty percent were rehabilitated successfully and resumed half-time or full-time work. The results of this study support the authors>> hypothesis that treatment with staphylococcus toxoid may be a fruitful strategy in patients with fibromyalgia and chronic fatigue syndrome. Study Design. This is a prospective study designed to identify psychological factors associated with response to spinal cord stimulation (SCS) trial. Summary of Background Data. ⋯ Conclusions. Patient mood state is an important predictor of trial outcome. Specific indicators of SCS trial outcome are the MMPI depression and mania subscale scores with successful trials being associated with individuals who are less depressed and have higher energy levels.

Objective. To evaluate the effectiveness of spinal cord stimulation using multiple independent programmable electrode selections compared to simple continuous stimulation. Design. ⋯ Conclusions. Continuous stimulation was not selected by any patient in favor of multi-stimulation or patient-controlled stimulation. This study indicates that in spinal cord stimulation the use of multiple electrodes together with advanced programmability increases paresthesia overlap, reduces pain scores, reduces revision rates, and improves patient satisfaction with spinal cord stimulation therapy.
