Randomized Controlled Trial Multicenter StudyMotion style acupuncture treatment (MSAT) for acute low back pain with severe disability: a multicenter, randomized, controlled trial protocol.
Acupuncture is widely-used to treat patients with low back pain, despite insufficient evidence of the technique's efficacy for acute back pain. Motion style acupuncture treatment (MSAT) is a non-traditional acupuncture treatment requiring a patient to exercise while receiving acupuncture. In Korea, MSAT is used to reduce musculoskeletal pain and improve functional status. The study aims to evaluate the effect of MSAT on acute low back pain with severe disability. ⋯ The results of this trial will be discussed.

Inflammation is a well-established contributor to numerous medical conditions, and has discrete roles in disease processes ranging from coronary artery disease and end-stage renal disease to both acute and chronic pain conditions. In some disease processes, inflammatory biomarkers are used clinically to track disease progression and efficacy of treatment, as is seen with complex regional pain syndromes and coronary artery disease. The recognition of such inflammatory biomarkers, and the use of immunomodulating agents for the treatment of chronic pain conditions has become an area of interest, due in part to the successful use of immunomodulating therapy not only for disease progression, but also for symptomatic relief in patients with chronic inflammatory conditions. This article will review this topic and the evidence related to immunomodulation for chronic pain conditions.

The findings of an expert panel convened to review critically how best to apply evidence-based guidelines for the treatment of acute pain in the Middle East region are presented. The panel recommended a three-step treatment protocol. Patients with mild-to-moderate levels of acute pain should be treated with paracetamol (step 1). ⋯ Patients reporting severe pain should be referred to a pain clinic or specialist for opioid analgesic treatment. Measures of pain and functioning that have been validated in Arabic, with culturally appropriate and easy to understand descriptors, should be used. Early and aggressive acute pain management is important to reduce the risk of pain becoming chronic, especially in the presence of neuropathic features.

Acute low back pain is marked by variability: An internet-based pilot study.
Pain variability in acute LBP has received limited study. The objectives of this pilot study were to characterize fluctuations in pain during acute LBP, to determine whether self-reported 'flares' of pain represent discrete periods of increased pain intensity, and to examine whether the frequency of flares was associated with back-related disability outcomes. ⋯ Acute LBP is characterized by variability. Patients with acute LBP report multiple distinct flares of pain, which correspond to discrete increases in pain intensity. A higher flare frequency is associated with worse disability outcomes.

Diclofenac epolamine topical patch relieves pain associated with ankle sprain.
Sports-related injuries, such as sprains and strains, commonly occur during exercise and athletic events. Current therapy includes nonsteroidal anti-inflammatory drugs (NSAIDs), which have a high incidence of upper gastrointestinal side effects. The present study assessed the efficacy and safety of the diclofenac epolamine topical patch (DETP, 1.3%), a topical NSAID for the treatment of acute minor sprains and strains. ⋯ Overall, the results of this study demonstrate that the DETP is an effective analgesic for local treatment of pain in mild acute ankle sprain.
