Ethnic minority groups in England have been disproportionately affected by the COVID-19 pandemic and have lower vaccination rates than the White British population. We examined whether ethnic differences in COVID-19 mortality in England have continued since the vaccine rollout and to what extent differences in vaccination rates contributed to excess COVID-19 mortality after accounting for other risk factors. ⋯ Lower COVID-19 vaccination uptake in several ethnic minority groups may drive some of the differences in COVID-19 mortality compared to White British. Public health strategies to increase vaccination uptake in ethnic minority groups would help reduce inequalities in COVID-19 mortality, which have remained substantial since the start of the vaccination campaign.

Sociodemographic factors have been found to be associated with the transmission of coronavirus disease 2019 (COVID-19), yet most studies focused on the period before the proliferation of vaccination and obtained inconclusive results. In this cross-sectional study, the infections, deaths, incidence rates, case fatalities, and mortalities of Virginia's 133 jurisdictions during the pre-vaccination and post-vaccination periods were compared, and their associations with demographic and socioeconomic factors were studied. The cumulative infections and deaths and medians of incidence rates, case fatalities, and mortalities of COVID-19 in 133 Virginia jurisdictions were significantly higher during the post-vaccination period than during the pre-vaccination period. ⋯ Multiple linear regression analysis suggested that demographic and socioeconomic factors contributed up to 80% of the variation in the infections, deaths, and incidence rates and up to 53% of the variation in the case fatalities and mortalities of COVID-19 in Virginia. The demographic and socioeconomic determinants differed during the pre- and post-vaccination periods. The developed multiple linear regression models could be used to effectively characterize the impact of demographic and socioeconomic factors on the infections, deaths, and incidence rates of COVID-19 in Virginia.

Multisystem inflammatory syndrome in children (MIS-C), which is associated with coronavirus disease 2019 (COVID-19) and occurs in the immediately post-infectious period, has never-before been reported within the active duty population. It typically affects children, aged 5-13 years, but has been shown to affect those up to 20 years old. We present an 18-year-old active duty male that arrived at a military treatment facility emergency department with headache, neck pain, and shock without evidence of meningoencephalitis on cerebrospinal fluid analysis and with a negative COVID-19 test. ⋯ It is a rare but potentially fatal condition that has been shown to affect patients up to the age of 20, encompassing a large part of the junior enlisted population. Multisystem inflammatory syndrome in children (MIS-C) can lead to death, yet mimic other diseases leading to delay of care. Thus, it should be considered when faced with the appropriate constellation of symptoms.

Immunocompromised patients may be at higher risk of mortality if hospitalised with Coronavirus Disease 2019 (COVID-19) compared with immunocompetent patients. However, previous studies have been contradictory. We aimed to determine whether immunocompromised patients were at greater risk of in-hospital death and how this risk changed over the pandemic. ⋯ Immunocompromised patients remain at elevated risk of death from COVID-19. Targeted measures such as additional vaccine doses, monoclonal antibodies, and nonpharmaceutical preventive interventions should be continually encouraged for this patient group.

ReviewClinical usefulness of testing for severe acute respiratory syndrome coronavirus 2 antibodies.
In the COVID-19 pandemic era, antibody testing against severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has proven an invaluable tool and herein we highlight some of the most useful clinical and/or epidemiological applications of humoral immune responses recording. Anti-spike circulating IgGs and SARS-CoV-2 neutralizing antibodies can serve as predictors of disease progression or disease prevention, whereas anti-nucleocapsid antibodies can help distinguishing infection from vaccination. ⋯ Additional crucial applications include population immunosurveillance, either at the general population or at specific communities such as health workers. Finally, we discuss how testing of antibodies in cerebrospinal fluid can inform us on the neurological complications that often accompany COVID-19.
