Case ReportsManagement of upper airway obstruction in the intensive care unit.
Three patients under observation in ICUs developed mechanical upper airway obstruction requiring surgical intervention for airway control. One patient was treated by tracheotomy placed through a vertical midline incision and 2 patients were treated by cricothyroidotomy followed by elective tracheostomy and closure of the cricothyroidotomy. ⋯ The importance of rapid cricothyroidotomy through a small incision in patients unresponsive to less invasive measures is emphasized. The use of a small pediatric endotracheal tube placed through the cricothyroidotomy incision is suggested as a method to increase the speed and safety of the procedure and avoid the potential long-term complication of laryngeal stricture.

Twenty-eight articles representing 19 investigations of critical care unit stress were reviewed. Studies were chosen if the stated purpose was to explore, describe, categorize, reduce, compare, or manipulate stress experienced by nurses employed in critical care settings. types of critical care units included were the intensive care unit, coronary care unit, pediatric intensive care unit, and the neonatal intensive care unit. ⋯ Theoretical bases, implementation of interventive strategies, and evaluation of interventions were absent in all studies reviewed. Recommendations for further research directions are given, and a categorization scheme for classification of perceived stresses is offered.

[Thromboembolic complications in resuscitation and intensive care. Clinical and therapeutic considerations].
The Authors, on the ground of the data of the experience of the Reanimation and Intensive Care Department (Palermo University), emphasize that the patients admitted in these units are "high risk patients" for thrombosis and embolism diseases for the invasive techniques used, for stillness, shock and politrauma. The Authors recommend an adequate prophilaxis also if often very difficult and emphasize that using Urokynase and Calcium-Heparin recovers the illness of thrombo-embolic complications also if prognosis remain non modified in the pulmonary general form.
