Because of an explosive increase in the use of emergency rooms in hospitals across the country, many hospitals have found it expedient to employ full-time emergency physicians. The demand for emergency physicians has prompted the development of training programs in emergency medicine in academic medical centers. ⋯ With the increasing identification and distribution of emergency physicians, there has been an attempt to develop emergency medicine as a distinct discipline and to establish its legitimacy in the structure of academic medical centers. A variety of problems must be addressed to clarify the role of emergency medicine in the academic setting.

Implementation of legislative requirements for emergency medical services in prepaid group practice organizations.
The Health Maintenance Organization Act of 1973, the Emergency Medical Services (EMS) Systems Act of 1973, and other laws are examined for their effects on the organization and management of emergency services in prepaid group practice plans (PPGP). The study was conducted in 1974-75 by the Group Health Association of America. The data were gathered through interviews with administrators and providers of seven PPGPs and with leaders of health planning agencies in the same communities, as well as through reviews of internal documents and a 1-month utilization survey of emergency and urgent care services in each PPGP. ⋯ As a result, the utilization of expensive emergency care must be carefully controlled, and this restraint is often accomplished by requirements specifying which health problems are appropriate for the provision of emergency care, rather than by delaying assistance until the plan's office hours. The furtherance of the PPGP concept, that the entire health care of the individual person is provided and financed by one organization, definancedby one organization, detracts from the viability of a central body charged with the coordination of the delivery of all emergency services in the community. It results not only in duplication of effort but often in the establishment of potentially antagoistic organizations.

A self-instructional program in emergency medicine has been developed for freshmen medical students at the University of Virginia School of Medicine. The cognitive objective of the course is to give the student the minimum level of knowledge to diagnose emergency medical conditions. ⋯ Self-instructional guides, algorithms, videotapes and reading materials are the educational resources for the course. As a result of successful completion of the training program, the student is certified as an emergency medical technician in the Commonwealth of Virginia.

The four-year program for training physicians in critical care-emergency medicine at Sacramento Medical Center, University of California, Davis School of Medicine is described. Its purpose is to train the physician in resuscitation, emergency care for life-threatening conditions, and intensive care, so that the graduate can staff community hospital emergency departments and intensive care units, be director of an emergency department or intensive care unit, or pursue an academic career in emergency medicine.
