Oropharyngeal and Tongue Pulse Oximetry in 2 Critically Ill Pediatric Patients: A Case Report.
Pulse oximetry has become a standard of care to monitor oxygenation. Absent or inaccurate readings can occur with varied patient states. We present preliminary experience with a modification of a standard pulse oximetry using readily available equipment (oral airway and a tongue blade) to allow for continuous pulse oximetry from the oral cavity and tongue in 2 critically ill pediatric patients when standard application of pulse oximetry was not feasible or nonfunctional. These modifications can assist in the care of critically ill patients, allowing for adaptability in monitoring techniques when other options are unavailable.

Lower serum chloride concentrations are associated with an increased risk of death in ICU patients with acute kidney injury: an analysis of the MIMIC-IV database.
The association between baseline serum chloride levels and mortality in patients with severe acute kidney injury (AKI) is unknown. Our aim was to investigate the relationship between baseline blood chloride levels and 28-day mortality in patients with AKI admitted to the ICU and to detect possible effect modifiers in this population. ⋯ Low serum chloride levels at baseline were associated with death at 28 days in intensive illnesses with AKI.

Randomized Controlled TrialRandomization to a Liberal Versus Conservative Oxygenation Target: Redox Responses in Critically Ill Children.
Optimal systemic oxygenation targets in pediatric critical illness are unknown. A U-shaped relationship exists between blood oxygen levels and PICU mortality. Redox stress or iatrogenic injury from intensive treatments are potential mechanisms of harm from hyperoxia. ⋯ On comparing liberal with conservative oxygenation targets, we show, first, significant redox response (increase in urinary markers of RONS), but no changes in markers of lipid or protein oxidation. We also show what appears to be an early hypoxic response (increase in HIF-1a gene expression) in subjects exposed to conservative rather than liberal oxygenation targets.
