Canalith repositioning maneuver for benign paroxysmal positional vertigo: randomized controlled trial in family practice.
To determine whether the canalith repositioning maneuver (CRM) is effective for treating benign paroxysmal positional vertigo when it is used by family physicians in primary care settings. ⋯ A statistically significant proportion of patients in the CRM group returned to a negative response to the DH maneuver immediately after the first treatment. Family physicians can use the CRM to treat benign paroxysmal positional vertigo and potentially avoid delays in treatment and unnecessary referrals.

Efficacy of acupuncture on osteoarthritic pain. A controlled, double-blind study.
Forty patients, randomly assigned to an experimental and a control group, participated in a double-blind study to assess the effectiveness of acupuncture in reducing chronic pain associated with osteoarthritis. The experimental group received treatment at standard acupuncture points, and the control group at placebo points. ⋯ Thus, both experimental and control groups showed a reduction in pain after the treatments. These results may reflect the natural course of illness, and various attitudinal and social factors.

Non-insulin-dependent diabetes can be treated using two criteria, the fasting blood glucose concentration and body weight. The feasibility of this means of control, aiming for a fasting blood glucose concentration of less than 6 mmol/liter (108 mg/dl) has been investigated in 10 general practices. Eight practices now use the method routinely. ⋯ Six practices preferred to have special clinics at which fasting blood glucose meaurements were taken. Two practices achieved good results with the administration and tests being undertaken by a practical nurse, with supervision from the general practitioner. Assessment of control by four fasting blood glucose determinations a year is less expensive than regular urine tests, and may improve blood glucose control.

Exosomes and microvesicles (MVs) are nanometer-sized, membranous vesicles secreted from many cell types into their surrounding extracellular space and into body fluids. These two classes of extracellular vesicles are regarded as a novel mechanism through which cancer cells, including virally infected cancer cells, regulate their micro-environment via the horizontal transfer of bioactive molecules: proteins, lipids, and nucleic acids (DNA, mRNA, micro-RNAs; oncogenic cargo hence often referred to as oncosomes). ⋯ This current review offers an overall perspective on the roles of exosomes and MVs in HNC biology, focusing on EBV-associated NPC and OSCC. We also highlight the importance of saliva as a proximal and easily accessible bio-fluid for HNC detection, and propose that salivary vesicles might serve as an alternative model in the discovery of novel HNC biomarkers.
