AbstractSurgical management of early supraglottic cancer has evolved over the last half of the century from total laryngectomy to inclusion of conservation approaches. The development and fundamental principles underlying open supraglottic laryngectomy and endoscopic supraglottic resections are reviewed. In doing so, a rational approach for the application of these surgical techniques is provided for management of both the primary site and the neck.
