AbstractSurgical management of supraspinatus compartment syndrome has not been previously reported. A high index of suspicion for severe shoulder pain of unclear etiology coupled with MRI is necessary for diagnosis. Prompt decompression to avoid irreversible ischemia is warranted. Perhaps mild ischemia of the supraspinatus muscle is an unrecognized cause of recurrent shoulder pain.
