AbstractHeather was drawing up antibiotics which were to be given intravenously to a patient suffering from a severe chest infection. She had to mix the antibiotic with a saline solution and then administer it to the patient. She had a lot of patients to care for and failed to notice that the expiry date on the antibiotic had passed. The patient subsequently suffered harm. Is Heather to blame?
