AbstractMRI plays a pivotal role in the staging of early rectal cancer, resulting in allocation of patients to surgery or organ-sparing treatment. In this large population-based retrospective cohort study, MRI substantially understaged pT3 and overstaged pT1 rectal cancer, in addition to unreliable nodal staging. Based on these findings, MRI is not adequate in allocating patients with rectal cancer to organ-sparing treatment.
