    AbstractAn anaphylactic reaction to protamine sulphate during cardiac surgery is a rare but known entity. Preoperative prediction and outcome of such a reaction is still unclear. We report a case of severe anaphylactic reaction to protamine sulphate in a diabetic while performing beating heart bypass surgery. We also reviewed the literature and discussed the management of such cases.
