    AbstractIncorrect Author Affiliation: In the article titled “Effect of Human Papillomavirus on Patterns of Distant Metastatic Failure in Oropharyngeal Squamous Cell Carcinoma Treated With Chemoradiotherapy,”published online March 5, 2015, and also in this issue of JAMA Otolaryngology–Head & Neck Surgery (doi:10.1001/jamaoto.2015 .136), the Author Affiliations were incorrect. That section should have been given as follows: "Author Affiliations: Head and Neck Institute, Cleveland Clinic Foundation, Cleveland, Ohio (Trosman, Lamarre, Scharpf, Khan, Lorenz, Burkey); Department of Radiation Oncology, Taussig Cancer Institute, Cleveland Clinic Foundation, Cleveland, Ohio (Koyfman,Ward, Greskovich); Department of Otolaryngology–Head and Neck Surgery, Rush University, Chicago, Illinois (Al-Khudari); Department of Hematology and Medical Oncology,Taussig Cancer Institute, Cleveland Clinic Foundation, Cleveland, Ohio (Nwizu, Adelstein)." This article was corrected online and in print.
