    Purpose Of ReviewTo provide a framework for addressing the social determinants of health (SDH) in the adolescent medical home.Recent FindingsThere has been recent recognition worldwide that SDH affect health throughout the life-course. Adolescents are a vulnerable population, with health behaviors, access, and outcomes shaped by social and environmental contexts. Medical homes have aimed to reduce the health inequities by facilitating connections with community resources. Interventions have been described in the literature for triaging social needs, connecting patients to resources, and utilizing multidisciplinary team members.SummarySDH should be incorporated into the routine adolescent healthcare. Screening tools and referral programs should be developed and evaluated in the adolescent population. Programs utilizing multidisciplinary team members would be effective in reaching adolescents and facilitating connections to resources.
