AbstractStreptococcus pyogenes is a rare cause of mastoiditis in adults. We report a previously healthy adult patient who had acute bacteraemic S. pyogenes mastoiditis complicated by intra-cranial air. This novel report adds pneumocephaly to the currently recognized list of complications of acute mastoiditis.
