Review Case Reports[Coronary embolism: a not so rare cause of myocardial infarction? Review of the literature about five suspected cases of patients in atrial fibrillation].
Five cases of myocardial infarction suspected to be due to coronary embolism are presented. All five patients had atrial fibrillation (AF), four of them with nonvalvular AF. The literature regarding coronary embolism is reviewed; the clinical manifestations and the place of AF are discussed.

Assessment of heart failure and left ventricular systolic dysfunction after cardiac pacing in patients with preserved left ventricular systolic function.
There is an accumulating data suggesting the deleterious effects of right ventricular pacing on left ventricular performance. Such pacing mimics left bundle branch block resulting in a prolonged QRS duration and causes ventricular asynchrony. ⋯ Patients with atrioventricular block and preserved left ventricular systolic function at baseline decrease significantly left ventricular ejection fraction after pacing. Induced ventricular asynchronism plays a major role in the deterioration of left ventricular function. Prolonged paced QRS width is a good predictor of left ventricular dysfunction after pacing. Larger prospective studies are needed to confirm these data.

[Acute pericarditis in the modern era: a diagnostic challenge].
Diagnosis of acute pericarditis remains difficult in clinical practice. ⋯ Clinical spectra of acute pericarditis have changed and some classic assumptions and descriptions, perpetuated in some publications, are outdated. Clinical presentation implies a 45-55 year-old man, with a chest pain and ST-segment elevation, without fever or pericardial friction rub, and a positive cTnI test in 27% of cases. Therefore, misinterpretation as other disease, especially acute myocardial infarction, is common and diagnosis of acute pericarditis remains often retrospective. In our series, a cTnI rise did not appear as a negative prognostic marker.

Case Reports[Evolution of the surgical strategy in hypertrophic cardiomyopathy: case studies of eight patients].
Hypertrophic cardiomyopathy is an inherited disease characterized by a left ventricular hypertrophy, a diastolic dysfunction and rhythm troubles with risk of sudden death. There was an evolution in the surgical strategy to treat the patients who present a left ventricular outflow tract gradient. ⋯ Surgical management of obstructive hypertrophic cardiomyopathy remain an important option in young patients, in case of failure of the ethanol septal ablation or in patients who present other surgical lesions. The dual chamber stimulation remain indicated in old patients.

Comparative Study[Antifibrinolytic agents and repeat open-heart surgery: a comparison of tranexamic acid and low dose aprotinin].
This retrospective study has for objective to compare the effect of tranexamic acid with low-dose of aprotinin therapy on blood loss in reoperative cardiac surgery. ⋯ This study concludes that tranexamic acid and low dose aprotinin effectively reduces postoperative bleeding in repeat open-heart surgery. However, the marked difference in superiority between these tow drug therapies needs the randomized and controlled study.
