Number of Prehospital Defibrillation Shocks and the Return of Spontaneous Circulation in Out-of-Hospital Cardiac Arrest.
It has not been determined yet whether the number of defibrillation shocks delivered over the first 30 min of cardiopulmonary resuscitation (CPR) impacts the rate of successful return of spontaneous circulation (ROSC) in out-of-hospital cardiac arrest (OHCA). ⋯ The delivery of four defibrillation shocks in OHCA most related to ROSC. The evaluation of the number of delivered shock during the first 30 min of CPR is a simple tool that can be used for an early decision in OHCA patient.

Association of Low Blood Pressure, Low Bispectral Index and Low Minimum Alveolar Concentration of Anaesthetic during Surgery with Postoperative 30-day Mortality: A Systemic Review and Meta-Analysis.
The triple low state [low mean arterial pressure, low bispectral index (BIS) and a low minimum alveolar concentration (MAC)] fraction of anaesthetic during surgery) has been a controversial subject of interest in clinical practise. Previous retrospective studies have produced different conclusions on the association between a triple low state and postoperative 30-day mortality. This study was a systematic review of previous studies on the effects of the triple low state on postoperative 30-day mortality and a meta-analysis with a sample size larger than that of previous studies. ⋯ The present study demonstrated that patients exposed to the triple low state exhibit higher 30-day mortality rates than those not exposed to the triple low state.

Effective Loading Dose of Dexmedetomidine to Induce Adequate Sedation in Parturients Undergoing Caesarean Section Under Spinal Anaesthesia.
To calculate the effective dose (ED) of dexmedetomidine for caesarean section under spinal anaesthesia. ⋯ The ED50 and ED95 of loading dexmedetomidine to achieve adequate sedation were 0.82 μg kg-1 and 0.96 μg kg-1 for caesarean section under spinal anaesthesia.

Case ReportsUnanticipated Difficult Airway in a Neonate: Are we Prepared for this CHAOS?
Unanticipated difficult airway in a neonate is a challenging situation with many difficulties because of inherent anatomical variations. To complicate the situation there is a lack of appropriate equipment, expertise and established guidelines on the management of difficult airway in neonates and infants. ⋯ We report the case of airway management of a neonate with an unanticipated finding of subglottic stenosis. Subglottic stenosis is one of the aetiologies of congenital high airway obstruction syndrome, which may be diagnosed antenatally based on ultrasonography findings.

Effect of Low Dose Remifentanil on Postoperative Pain Relief and Heart Rate Variability in Post-Anaesthesia Care Unit.
Several reports have shown the negligible adverse effects of low-dose remifentanil on the autonomic nervous system. We propose that the administration of low-dose remifentanil would be beneficial without adverse respiratory and hemodynamic effects. This study aimed to examine the effects of low-dose remifentanil on postoperative pain relief and heart rate variability (HRV) after surgery. ⋯ The continuous infusion of low-dose remifentanil may reduce post-operative pain scores and trigger the relative activation of the parasympathetic nervous system in post-surgical patients. This indicates that continuous infusion of low-dose remifentanil may be a useful option for postoperative pain relief in cases where postoperative pain control proves inadequate even with the application of regional block technique.
