Vaginal birth after cesarean in California: before and after a change in guidelines.
In 1999 the American College of Obstetricians and Gynecologists (ACOG) adopted more-restrictive guidelines for vaginal birth after cesarean delivery (VBAC). This study assesses trends in VBAC in California and compares neonatal and maternal mortality rates among women attempting VBAC delivery or undergoing repeat cesarean delivery before and after this guideline revision. ⋯ Neonatal and maternal mortality rates did not improve despite increasing rates of repeat cesarean delivery during the years after the ACOG 1999 VBAC guideline revision. Women with infants weighing > or =1,500 g encountered similar neonatal and maternal mortality rates with VBAC or repeat cesarean delivery.

The US Medical Liability System: evidence for legislative reform.
Despite state and federal efforts to implement medical malpractice reform, there is limited evidence on which to base policy decisions. The National Practitioner Data Bank (NPDB) offers an opportunity to evaluate the effects of previous malpractice tort reforms on malpractice payments and premiums. ⋯ Significant reductions in malpractice payments could be realized if total or noneconomic damage caps were operating nationally. Hard noneconomic damage and total damage caps could yield lower premiums. If tied to a comprehensive plan for reform, the money saved could be diverted to implement alternative approaches to patient compensation or be used to achieve other systems reform benefiting patients, employers, physicians, and hospitals.

Comparative StudyHow primary care networks can help integrate academic and service initiatives in primary care.
Theory of effective network operation in primary care is underdeveloped. This study aimed to identify how primary care networks can best integrate academic and service initiatives. ⋯ Networks can help integrate academic research and service development initiatives by facilitating interorganizational interactions and in shared leadership of projects. Researchers and practitioners stand to gain considerably from an integrated approach in both the short and the long term. Success requires agreement about a set of pathways, learning spaces, and feedback mechanisms to harness the insights and efforts of stakeholders throughout the whole system.

Development of a measure to assess patient trust in medical researchers.
Many researchers find it difficult to recruit individuals, particularly minorities, for participation in studies. Mistrust of research and researchers may act as a barrier to participation. The purpose of this study was to develop a scale for assessing trust in medical researchers. ⋯ The Trust in Medical Researchers Scale has good psychometric qualities and differentiates African American from white respondents, as well as individuals who indicate that they are likely to participate in medical research from those who are not. More focused investigations in hard-to-recruit populations will help to establish the utility of the Trust in Medical Researchers Scale.

Review Meta AnalysisEfficacy and safety of inhaled corticosteroids in patients with COPD: a systematic review and meta-analysis of health outcomes.
We wanted to review systematically the efficacy, effectiveness, and safety of inhaled corticosteroids with respect to health outcomes in patients with chronic obstructive pulmonary disease (COPD). ⋯ Overall, the risk-benefit ratio appears to favor inhaled corticosteroid treatment in patients with moderate to severe COPD. Existing evidence does not indicate a treatment benefit for patients with mild COPD.
