A novel influenza H1N1 began in March 2009, rapidly spread, and then became a pandemic outbreak. Diagnosis by polymerase chain reaction result was not always available because of a surge in workload and therefore clinical diagnosis became important. However, clinical differences between the patients infected by the novel H1N1 virus and those infected by the influenza-like non-novel H1N1 have not been reported. This study was conducted to compare the demographic background, clinical manifestations, and laboratory findings between novel H1N1 influenza infections and other non-novel H1N1 infections. ⋯ When a patient presents with influenza-like acute febrile respiratory illness symptoms and is young in age, has a travel history involving an affected area, and is suffering from myalgia or leukopenia, physicians should be alerted to the possibility of novel H1N1 virus infection.

Lamotrigine for trigeminal neuralgia: efficacy and safety in comparison with carbamazepine.
Anticonvulsants are regarded as useful for the treatment of neuropathic pain. In this study, we evaluated the efficacy and occurrence of side effects of lamotrigine (LTG) in comparison with carbamazepine (CBZ), in trigeminal neuralgia (TN) patients. ⋯ LTG is generally an effective and safe treatment for management of TN, compared to CBZ.

Clinical competency certifications are important parts of internal medicine residency training. This study aims to evaluate a composite objective structured clinical examination (OSCE) that assesses postgraduate year-1 (PGY(1)) residents' acquisition of the six core competencies defined by the Accreditation council for Graduate Medical Education (ACGME). ⋯ By using a structured development process, the authors were able to create reliable evaluation items for determining PGY(1) residents' acquisition of the ACGME core competencies.

Our aim here was to explore treatment efficacy of pemetrexed and docetaxel in non-small-cell lung cancer patients who had failed previous chemotherapy and epidermal growth factor receptor-tyrosine kinase inhibitor therapy. ⋯ This retrospective study of pemetrexed and docetaxel showed relatively safe toxicity profile, reasonable response rate, and long survival when used as third- and fourth-line chemotherapy. Thus, it is reasonable to give good performance status patients third- and fourth-line chemotherapy. A phase III randomized trial is needed for better clarification of these issues.

Systemic and fatal arterial air embolism during the computed tomography (CT) scan is rarely reported in English-based literature. Iatrogenic air embolism happening during the CT scan is often related to the injector, usually venous air embolism and asymptomatic. ⋯ The mechanism is different from the reported cases in the literature. The possible mechanism and pathogenesis are well discussed to alert clinicians and prevent the recurrence of such complication.
