A right adrenal tumor was found incidentally during abdominal computed tomography exam in a 51-year-old female patient, who had had diabetes and hypertension for more than 10 years. The computed tomography scan was arranged for possible pancreatic lesion by a neurologist. Norepinephrine level was high in the plasma and urine. ⋯ Therefore, adrenal cortical adenoma with subclinical Cushing's syndrome was highly suspected; however, further imaging studies, including magnetic resonance image and 131I-6beta-iodomethylnorcholesterol adrenal scintigraphy failed to discriminate an additional tumor. After right adrenalectomy, a small adrenal cortical adenoma and a large pheochromocytoma were noted. This is an extremely rare case of an adrenal incidentaloma consisting of both medullary and cortical tumors in the same gland.

Failure to withdraw a catheter is related primarily to catheter entrapment or knotting. Placement of a Swan-Ganz catheter is associated with various complications, one of which is entrapment. ⋯ Here we report a rare case of Swan-Ganz catheter entrapment resulting from inserting an excessive length of Swan-Ganz catheter into the heart. The catheter was removed smoothly under fluoroscopic guidance.

Fertility preservation for a patient with advanced immature teratoma of the ovary is reported. The patient, a 29-year-old woman, delivered a healthy baby after having had ovarian immature teratoma, grade 3, uncertain stage, at 13 years of age. ⋯ The patient delivered a healthy baby 16 years after the initial treatment. Based on this successful case, intensive fertility-preserving surgery followed by chemotherapy, even in advanced-stage immature teratomas of the ovary, may be effective in preserving the reproductive function of women with malignant immature teratomas of the ovary.

Chronic hepatitis B virus (HBV) infection is the primary risk factor for the development of hepatocellular carcinoma worldwide. After decades of chronic hepatitis, about 30-40% of patients progress into liver cirrhosis, and of them, around 1-5% subsequently develop hepatocellular carcinoma (HCC) annually. ⋯ Studying the genetic factors predisposing hepatocarcinogenesis in both host and viral genomes will help illuminate the critical carcinogenic mechanisms, and create molecular targets for future therapy. In this article, we thus review the epidemiology of HBV-related HCC and viral factors involved in hepatocarcinogenesis.

The number of publications in journals indexed in the Institute for Scientific Information (ISI) database of the Thomson Corporation is generally used to assess the research performance of individuals, institutions and countries in scientific fields. The aim of this study was to analyze the trends in Taiwan's ISI publications in clinical medicine from 1990 to 2004. Special attention was paid to internationally collaborated works that were identified based on the countries of co-authors' affiliations. ⋯ Taiwan has achieved a significant increase in the number of ISI publications in clinical medicine. Yet there exists opportunity for improvement in international collaboration.
