Evaluation of pharmacists' barriers to the implementation of medication therapy management services.
To assess pharmacists' actual and perceived barriers to implementing medication therapy management (MTM) services in the outpatient setting and to assess demographic and other factors associated with identified barriers. ⋯ These results show that the most important barriers to implementing MTM services in the outpatient setting identified by pharmacist survey respondents were related to interprofessional relationships, documentation, and compensation. Despite the resources available to pharmacists, barriers continue to hinder the expansion of MTM and direct patient care services.

Analysis of community chain pharmacists' interventions on electronic prescriptions.
To measure the incidence and nature of prescribing errors on electronic prescriptions (e-prescriptions) that required active intervention by dispensing pharmacists to correct. ⋯ Electronic prescribing can improve the safety and effectiveness of patient care. As currently implemented in the community practice setting, this still-emerging technology maintains selected threats to both medication safety and effectiveness, although probably less than handwritten prescriptions. The adoption of selected best practice recommendations by prescribers could improve the safety, effectiveness, and efficiency of e-prescribing.
