Phenothiazine-type antipsychotics elicit cutaneous analgesia in rats.
Local anesthetics exert their anesthetic and analgesic effects by blocking the sodium channels in the nervous system. Phenothiazine-type antipsychotics also block sodium channels, but the local anesthetic characteristics of these drugs have not been evaluated. The aim of this study was to evaluate the cutaneous analgesic effect of phenothiazine-type antipsychotics. ⋯ Of the antipsychotic drugs tested, mesoridazine is potentially the best candidate for development into a potent, long-acting local anesthetic. However, toxicity studies are needed before these agents can be used clinically as analgesics.

Case ReportsDifficult removal of an epidural catheter in the anterior epidural space.
When administering postoperative acute pain services, particularly regarding patient- controlled epidural analgesia, difficulties may occasionally be encountered during removal of the epidural catheter. In this report, we present an instance of difficult removal of epidural catheter in a female patient undergoing open reduction and internal fixation of the femoral neck with patient-controlled epidural analgesia as the means of postoperative pain control. The patient had satisfactory analgesia for 3 days; however, during the removal of the epidural catheter, difficulties were encountered and epidurogram revealed that the epidural catheter had become anchored in the anterior epidural space without kinking or knotting. ⋯ Fortunately, the epidural catheter was removed easily without the need for a guided stylet. We believe that the cause of the difficult removal of the epidural catheter in this case might have resulted from an unusual and unwanted deeper anchorage of the catheter along the anterior epidural space during placement. We also include some discussion on the management of problematic removal.

Case ReportsAn unusual case of peripartum cardiomyopathy in a parturient with preeclampsia.
Here we report an unusual development of peripartum cardiomyopathy (PPCM) in a parturient woman with preeclampsia. A 36-year-old nulliparous parturient woman underwent elective cesarean section for delivery of twins under spinal anesthesia. Both preoperative workup and past history were unremarkable except for proteinuria and hypertension for 1 week. ⋯ Fortunately, the patient recovered after treatment and was discharged 15 days later. This case illustrates that we should bear in mind the possibility of PPCM if orthopnea develops while delivery is approaching in a parturient with preeclampsia. Echocardiography is helpful for early diagnosis of PPCM.

Case ReportsApplication of a double-lumen tube for one-lung ventilation in patients with anticipated difficult airway.
One-lung ventilation (OLV) is essential in some surgical situations. The use of double- lumen tubes (DLTs) can achieve OLV more quickly and more easily than bronchial blockers. The management of a difficult airway is a challenge for anesthesiologists when, at the same time, OLV is needed for a surgical procedure. ⋯ Nasal intubation of Case 2 was done with fiberoptic-guided intubation with the patient awake. OLV was achieved uneventfully after inserting the DLT directly through the tracheostomy in both cases. We also describe the appropriate use of airway devices for OLV, focusing on patients with an anticipated difficult airway.

Case ReportsUse of the Rusch Flexi-Slip stylet for patients with difficult insertion of the ProSeal laryngeal mask airway.
The ProSeal laryngeal mask airway (PLMA) offers a more effective seal and is easier for gastric tube placement to prevent aspiration than the classical laryngeal mask airway. However, it is more difficult to insert with the digital and introducer tooling techniques. The Rusch Flexi-Slip stylet (RFSS) is an accessory intubation tool that consists of a malleable coated wire and a soft atraumatic tip. ⋯ The RFSS technique offers several advantages for PLMA insertion, including the provision of effective support and a soft tip for the PLMA insertion. Other techniques to facilitate the insertion of the PLMA, including priming the drain tube with a guide are discussed. We recommend that the RFSS technique offers an effective method for cases with difficult insertion of the PLMA.
