Inter-rater reliability of medication error classification in a voluntary patient safety incident reporting system HaiPro in Finland.
Medication errors are common in healthcare. Medication error reporting systems can be established for learning from medication errors and risk prone processes, and their data can be analysed and used for improving medication processes in healthcare organisations. However, data reliability testing is crucial to avoid biases in data interpretation and misleading findings informing patient safety improvement. ⋯ The data classified by different classifiers can be pooled for some of the medication error classes. Consistency of the classification and the quality of narratives need improvement, as well as reporting and classification of contributing factors to provide high quality information on medication errors.

Medication errors in community pharmacies: The need for commitment, transparency, and research.
Medication errors place a serious medical and economic burden on the United States (U. S.) healthcare system. Since 1975, government health agencies and non-profit organizations in the U. ⋯ Community pharmacies play a significant role in the U. S. healthcare system, but their efforts to reduce medication errors are not well known. By improving transparency in quality assurance processes and promoting patient engagement to improve patient safety, community pharmacies have the potential to play a more active role in reducing medication errors and safeguarding patients from harm.

Medication safety practices and methods for reporting errors in community pharmacies are relatively unknown. ⋯ This data supplements existing literature on medication safety practices and error reporting in community pharmacy settings, as well as highlights knowledge gaps outside the scope of this study.

FIP Perspectives: Realising global patient safety goals requires an integrated approach with pharmacy at the core.
Patient safety is becoming increasingly recognised as a top priority for action that requires a collective and coordinated response. The leading cause of harm or injury in health care systems is medication errors. ⋯ Pharmacists are charged with the responsibility to ensure that when a patient receives and uses a medicine, it will not cause harm or death. The International Pharmaceutical Federation (FIP) recognises the critical role the pharmacy plays in realising global, regional and national patient safety goals; and works with its partners, stakeholders and members around the world to advocate for the role of pharmacy in achieving this global patient safety agenda and to envision a world of safe access to medicines and care.

Strategies for improving medication safety in hospitals: Evolution of clinical pharmacy services.
Medication safety risks are the most important preventable factors jeopardizing patient safety. To manage these risks, extending pharmacists' involvement in patient care and patient safety work has been systematically addressed in patient safety initiatives since the early 2000s. ⋯ Pharmacists' involvement in patient care and system-based medication safety work was reported to become more common in Finnish hospitals during 2011-2016. This development is in line with patient safety policy initiatives and its impact on patient care outcomes should be followed up.
