Implications of COVID-19 on time-sensitive STEMI care: A report from a North American epicenter.
Coronavirus disease 2019 (COVID-19) has forced dramatic changes to the healthcare systems throughout the world. Time-sensitive management of cardiovascular emergencies such as ST-elevation myocardial infarction (STEMI) has yet to be evaluated in the context of these new policies, particularly in so-called "hot spot" cities. ⋯ During the lockdown period, many patients appear to have been reluctant to present to hospitals. This was associated with more unstable STEMI presentations and worse in-hospital course. Importantly, the health care system appears able to ensure timely acute cardiac care while ensuring that COVID-19 protocols are respected.

Case ReportsSuccessful thrombolysis of acute upper limb ischemia using ekosonic endovascular system ultrasound catheter in a patient with atrial fibrillation.
Acute upper limb ischemia is considered a vascular emergency. These events are mostly thromboembolic and are often detected in patients with atrial fibrillation (AF). Surgical thrombectomy and percutaneous mechanical thrombectomy are the usual methods to treat this medical emergency. ⋯ Then an EKOS Catheter was delivered to the occlusion area and was placed for 6 h. Symptoms of ischemia were resolved, and the angiographic control showed normal flow in the axillary artery. The patient was discharged without any local deficits of the right hand.

Case ReportsTransvenous retrieval of a pulmonary artery catheter knot around pacing leads: A case report.
Pulmonary Artery Catheter (PAC) knotting is a rare complication of PAC insertion. In patients with dilated right heart chambers, blind insertion of PAC significantly increases the risk of catheter knotting. We demonstrate a safe and successful approach to resolving a PAC knot around pacing leads of a cardiac resynchronization device. ⋯ To highlight a potential complication of blind pulmonary artery catheter insertion and provide a safe technique to resolve catheter knots.

Meta-analysis of direct oral anticoagulants compared with vitamin K antagonist for left ventricle thrombus.
The use of direct oral anticoagulants (DOAC) in preference to vitamin K antagonists (VKA) as a treatment of left ventricle (LV) thrombus is controversial. ⋯ In this pooled analysis, outcomes in patients on DOAC were comparable to VKA. The hypothesis generated could suggest DOAC could be used interchangeably with VKA in patients with LV thrombus. Randomized trials are needed for generalization of results.

We aimed to study the impact of frailty on the outcome of transcatheter aortic valve replacement (TAVR) procedures. ⋯ The HFRS can be utilized in the risk stratification of older patients undergoing TAVR.
