Influence of patient-related and surgery-related risk factors on cognitive performance, emotional state, and convalescence after cardiac surgery.
Cardiac surgery may account for complications such as cognitive impairment, depression, and delay of convalescence. This study investigated the influence of different risk factors on cognitive performance, emotional state, and convalescence. ⋯ For elderly patients undergoing cardiac surgery, older age, total preexisting medical risk factors, and surgery duration seem to be the most important factors influencing cognitive outcome and convalescence. Results show that, also for patients without postoperative delirium, medical risk factors and intraoperative parameters can result in delay of convalescence.

Case ReportsLeft-circumflex coronary artery to right atrium fistula with saccular aneurysm and its endovascular treatment.
The combination of coronary artery aneurysm and coronary artery fistula is infrequent. A saccular aneurysm of a branch of the left-circumflex coronary artery associated with multiple fistulae to the right atrium was observed on a coronary angiogram performed in a 47-year-old female. Multidetector computed tomography coronary angiography detailed the anatomy of the abnormal coronary artery. An embolization with a microcoil was performed and the aneurysm sac was excluded.

Percutaneous vertebral artery intervention: a necessary tool in every interventional cardiologist armamentarium.
The purpose of this study was to evaluate the results of endovascular treatment of symptomatic vertebrobasilar insufficiency unresponsive to medical therapy. ⋯ Posterior fossa ischemia is an underdiagnosed condition that occurs with relative frequency in the usual patient population that interventional cardiologists attend to. Endovascular treatment using coronary wires and stents including drug-eluting stents is the treatment of choice for vertebral artery revascularization due to its high technical success rate, low complication rate, and long-term durability.

Case ReportsCase report: a very large dissection in the left anterior descending coronary artery of a 56-year-old man.
Spontaneous coronary artery dissection (SCAD) is a rare condition that usually occurs in relatively young patients who are predominantly female. Seldom it could be a cause of acute myocardial ischemia leading to a sudden cardiac death. SCAD consists of intramural hematoma formation or, rarely, intimal tears that initiate and propagate the dissection in the vessel wall. ⋯ After a 2-year follow-up, there was absence of both angina and myocardial ischemia and there was a decrease in platelet count. Many strategies could be considered in patients with SCAD, such as PCI, bypass surgery, or conservative medical management. In general, the long-term prognosis of patients with SCAD is considered favorable if they survive the acute phase.

Comparative StudyUsing a 0.035-in. straight-tip wire and a small infant laryngoscope for safe and easy endotracheal intubations in rats for cardiovascular research.
Endotracheal intubation in rats is a challenging problem. This is a difficult procedure due to the narrow oral cavity and the difficulty in visualizing the vocal cord. It is associated with high complication rate. We have developed a very safe and simple method using a 0.035-in. straight tip wire as a guide and the small infantile laryngoscope for intubation in rats. ⋯ With a 0.035-in. straight-tip wire as a guide and a small infant laryngoscope blade Miller size 0, endotracheal intubations in rats can be safely performed with high success rate without the use of atropine, which makes it better suitable for cardiovascular research.
