Predicting all-cause readmissions using electronic health record data from the entire hospitalization: Model development and comparison.
Incorporating clinical information from the full hospital course may improve prediction of 30-day readmissions. ⋯ Incorporating clinically granular EHR data from the full hospital stay modestly improves prediction of 30-day readmissions. Given limited improvement in prediction despite incorporation of data on hospital complications, clinical instabilities, and trajectory, our findings suggest that many factors influencing readmissions remain unaccounted for. Further improvements in readmission models will likely require accounting for psychosocial and behavioral factors not currently captured by EHRs. Testing the functional assessment of mentation: A mobile application based assessment of mental status.
Altered mental status is a significant predictor of mortality in hospitalized patients and a prerequisite component to the diagnosis of delirium. However, the detection of altered mental status is often incomplete, inaccurate, and resource intensive. ⋯ FAM(TM) is a rapid and clinically feasible tool that can identify minor alterations in mental status often missed by GCS.
