[Oral appliances or maxillomandibular advancement osteotomy for severe obstructive sleep apnoea in patients refusing CPAP].
The management of patients with severe obstructive sleep apnea syndrome (OSAS) refusing or not tolerating continuous positive pressure ventilation (CPAP) remains problematic. We evaluated the effectiveness of oral appliances and of maxillomandibular advancement osteotomy. ⋯ We are confronted with an increasing number of severe OSAS patients with CPAP failure or intolerance. Surgery for maxillo-mandibular advancement is an effective alternative. However, it is not always indicated or accepted by the patient. So an oral appliance remains a useful therapeutic option despite its moderate success rate.

Clinical Trial[Botulinum toxin A: analgesic treatment for temporomandibular joint disorders].
Pain, along with clicking, sub-luxation, and blocking, is one of the main symptoms for patients presenting with temporomandibular joint disorders. We assessed the effectiveness of botulinum toxin A (BOTOX(®), Allergan) as analgesic treatment for temporomandibular joint disorders. ⋯ Botulinum toxin A significantly decreases pain and improves movements of patients presenting with temporomandibular joint disorders. The effects are prolonged three months after the injection.

Case Reports[Cephalic tetanus revealed by peripheral facial palsy].
Cephalic tetanus is a rare presentation of localized tetanus (1 to 3% of localized presentations). It is a very rare cause of peripheral facial palsy. ⋯ Facial peripheral palsy often appears after trismus. It is very rarely the first symptom of cephalic tetanus, making the diagnosis difficult.

Case Reports[Intractable epistaxis in Osler-Rendu-Weber syndrome].
Osler-Rendu-Weber syndrome is an inherited disorder characterized by mucocutaneous and visceral telangiectasia. It is often revealed by recurrent epistaxis. It is sometimes profuse and intractable. Hemostasis may be problematic. ⋯ Embolization of external carotid artery branches can be an effective therapeutic alternative when usual treatments fail.

Historical Article[From the first face replants operations to the first total face transplant: the history of surgical prowess].
Composite tissue allografts of the face were considered as science-fiction just a decade ago. They have become wonderful realities in our hospitals. Face transplantation is one of the great scientific adventures of the 21(th) century that history will remember. ⋯ In 2010, two teams performed the first total face transplants in the world. They represent technical milestones in the history of transplantation. These face transplants, which have raised a lot of controversy and ethical questions, open the way for other surgical perspectives, allowing medicine to write new history, and show that progress is also made through transgression.
