Randomized Controlled TrialEffects of different doses of intraumbilical oxytocin on the third stage of labor.
To determine the optimal dose of oxytocin to be injected intraumbilically after fetal delivery for active management of the third stage of labor. ⋯ Administering 30 IU of oxytocin intraumbilically in 50 mL of a normal saline solution after fetal delivery is a simple, noninvasive, and effective method for active management of the third stage of labor.

To review case reports of hepatic hematoma/rupture in women with pre-eclampsia/eclampsia. ⋯ HELLP syndrome is a frequent diagnosis (92.8%) in hepatic hemorrhage/rupture. The major reduction in maternal mortality rate was probably associated with advances in resuscitation, intensive-care medicine, and surgical intervention, including liver transplantation and arterial embolization.

Randomized Controlled TrialImpact of preoperative rectal misoprostol on blood loss during and after elective cesarean delivery.
To evaluate the effect of preoperative administration of rectal misoprostol on blood loss during and after elective cesarean delivery. ⋯ Preoperative treatment with 400 μg rectal misoprostol significantly reduced blood loss related to elective cesarean delivery.

Uterine artery, umbilical, and fetal cerebral Doppler velocities after epidural analgesia during labor.
To evaluate the effects of epidural analgesia on uterine artery, umbilical, and fetal cerebral Doppler velocities during labor. ⋯ The data suggest that the Doppler flow velocity of uterine arteries is affected by epidural analgesia during labor. Numerous Doppler flow studies of the effect of neuraxial blockade during labor on umbilical and uterine arteries have been published with incredibly variable and inconsistent results.
