Huge population of aggressively developing country like India is catered by more than 20,000 orthopedic surgeons but with very few dedicated foot & ankle surgeons. This area of orthopedics has not yet established its place as a specialty of medicine. Most of newer treatment concepts & methodologies are yet to establish its place in India in spite of huge suffering population. ⋯ Commonly seen forefoot, midfoot & hindfoot problems in India & their routine care is discussed. Article also addresses very tough but interesting journey of first foot & ankle surgeon of India & story of rise & fall of first Indian foot & ankle centre of excellence. Contribution in form of educational efforts & deep market penetration is deemed from west to uplift Indian foot & ankle scenario which is summed up as: lack of awareness, opportunity and negligence.

Case ReportsIsolated medial cuneiform fractures: report of two cases and review of the literature.
Although fractures of the midfoot are common, cuneiform fractures are rarely seen. These fractures are frequently associated with other fractures of the midfoot such as Lisfranc fracture-dislocations. However, isolated cuneiform fractures are extremely rare, with few cases reported in the relevant literature. ⋯ One of the patients was treated with headless screw fixation due to displacement in fracture configuration, and the other was treated conservatively. Fractures were united without any complication in both patients. In this report, the authors discuss the mechanism of injury, diagnostic challenges, and treatment options of isolated medial cuneiform fractures.

Case ReportsContracture of the third toe as a delayed presentation of a foreign body in the foot.
Foreign body retention in the foot after a penetrating injury is an unusual occurrence with varied presentation. A degree of suspicion needs to be present in situations where there is an inexplicable swelling, sinus, abscess, or tenderness. ⋯ This case report highlights the fact that an ultrasound to rule out retention of the foreign body might be a judicious investigation if common causes of toe contracture are excluded. It may also have therapeutic implications.

Case ReportsArthroscopic ankle arthrodesis after tibial pilon open reduction internal fixation.
Ankle arthrodesis is an essential tool in the foot and ankle surgeon's armamentarium. Despite the evolving technology and ongoing research in ankle arthroplasty, arthrodesis continues to be a proven and safe option for the majority of patients with ankle arthritis refractory to conservative management. Here, the authors present their technique of an arthroscopic ankle arthrodesis specifically in the setting of a previous open-reduction internal fixation (ORIF) for a tibia plafond type fracture. They have found this to be a reliable technique to achieve a solid ankle arthrodesis while minimizing soft-tissue trauma and dissection in an already compromised soft-tissue envelope.

ReviewThromboembolic prophylaxis in foot and ankle surgery what should we do?
The majority of orthopaedic venous thromboembolism (VTE) literature involves major orthopaedic surgery, which has distinct differences in comparison with foot and ankle surgery. Despite the current evidence-based medicine advocating against routine prophylaxis, confusion remains around who should be prophylaxed and with what modality. Clearly, larger prospective studies are needed to determine those individuals who require prophylaxis for the prevention of venous thromboembolic events following foot and ankle surgery.
