Association Between Overnight Extubations and Outcomes in the Intensive Care Unit.
Little is known about the timing of extubations for patients in the intensive care unit (ICU) who undergo mechanical ventilation (MV) or whether overnight extubation is safe. ⋯ Approximately one-fifth of patients with MV in US ICUs undergo overnight extubation. These patients have higher rates of ICU and hospital mortality than patients undergoing extubation during the daytime. Further studies are needed to understand why overnight extubation results in poorer outcomes.

Randomized Controlled Trial Multicenter StudyAzithromycin for Acute Exacerbations of Asthma : The AZALEA Randomized Clinical Trial.
Guidelines recommend against antibiotic use to treat asthma attacks. A study with telithromycin reported benefit, but adverse reactions limit its use. ⋯ In this randomized population, azithromycin treatment resulted in no statistically or clinically significant benefit. For each patient randomized, more than 10 were excluded because they had already received antibiotics.

Randomized Controlled Trial Multicenter Study Comparative StudyAssociation of Nausea and Vomiting During Pregnancy With Pregnancy Loss: A Secondary Analysis of a Randomized Clinical Trial.
Nausea and vomiting during pregnancy have been associated with a reduced risk for pregnancy loss. However, most prior studies enrolled women with clinically recognized pregnancies, thereby missing early losses. ⋯ Among women with 1 or 2 prior pregnancy losses, nausea and vomiting were common very early in pregnancy and were associated with a reduced risk for pregnancy loss. These findings overcome prior analytic and design limitations and represent the most definitive data available to date indicating the protective association of nausea and vomiting in early pregnancy and the risk for pregnancy loss.
