About 50% of all patients with pelvic fractures present with associated injuries. Diagnostic and therapeutic guidelines follow the step-by-step regimen used in polytraumatized patients. ⋯ Injuries of the pancreaticoduodenal system and ruptures of the digestive tract are particularly difficult to diagnose. Stabilization of pelvic fractures will take place in the phase of delayed reconstruction, usually several days after the accident.

Review Case Reports[A rare form of metastasis of renal cell cancer. A case report of intra-oral soft tissue metastasis].
Metastatic lesions represent 1%-8% of all malignant tumours of the mouth and jaws, which are regarded as rare sites of metastases from different primary tumours. The vast majority of these lesions (90%) have been observed in the mandibula, and 5%-20% in the maxilla. ⋯ The primary tumour that most commonly metastasizes to the mouth and jaws seems to be carcinoma of the lung, followed by breast cancer and renal cell carcinoma. The case of a 47-year-old woman with renal cell carcinoma and an intraoral soft tissue metastatic lesion is presented.

Transurethral resection (TUR) is a surgical operation generally performed in elderly patients, afflicted with polymorbidity and a high risk for anaesthesia. We studied 1351 transurethral resections during the years 1974-1984: 840 of these procedures were done under regional block--spinal as well as epidural--and 511 in general anaesthesia. Results and complications were related to the applied technique of anaesthesia. Considering that especially the very old patients belong to the regional anaesthesia group where general anaesthesia is known to be dangerous, the results of this study confirm definitely that regional anaesthesia is at present the most appropriate technique for TUR surgery.

[Peridural opioid analgesia in the terminal stage of cancer. Indications--implementation--results].
The indications for the application of opioids near the spinal cord are presented and two groups of patients treated with pericutaneous and intracorporeal systems are reported. If indicated, this method offers the possibility of treating patients on an outpatient basis.

Case Reports[Ureteral injuries in intervertebral lumbar disk surgery].
In lumbar disc surgery ureteral transection is a rare complication and only nine such cases have been reported so far. They are analyzed and two more cases are described. The complication in the first case was recognized immediately and the patient was treated successfully by end-to-end anastomosis of the ureter. The diagnosis in the second case was delayed and this lead to a septic course and finally required nephrectomy.
