A case of pediatric northern black widow spider (Latrodectus variolus) bite in New York, USA.
Black widow spiders (Latrodectus spp.) are generally perceived as inhabiting southern or western United States. However, some species, such as the northern black widow, Latrodectus variolus, are also found in more temperate regions of USA and adjacent Canada. ⋯ Medical practitioners often fail to recognize the signs and symptoms of latrodectism delaying effective treatment. Even in more northern locales, healthcare workers should take black widow spider envenomation under consideration in the differential diagnosis particularly with suspected exposure.

The Brown Widow spider (Latrodectus geometricus) is an invasive species whose geographic range has been expanding worldwide. It is a relative species of the Black Widow and Red-backed spiders of the genus Latrodectus. ⋯ Specific FDA approved Latrodectus antivenom (IgG) available in North America has been effectively used in treating venom-induced symptoms following L. mactans envenoming. The patient reported here involved a confirmed L. geometricus envenoming who was efficaciously treated with an alternately available F(ab')2 antivenom from Mexico.

Analgesic mechanism of Botulinum toxin type A (BoNT/A) involves retrograde axonal transport to central nervous system, where it may interact with sensory neurons. Though, some authors suggested that BoNT/A antinociceptive action may also be associated with the inhibition intracellular factors and neuromodulators expressed by immune cells, especially by microglia. Antigen-induced arthritis in the temporomandibular joint (TMJ) of rats is signal by P2X7 receptor/Cathepsin S (CatS)/Fractalkine (FKN) microglia-activated pathway. ⋯ Additionally, BoNT/A significantly reduced the protein levels of CatS, FKN, and TNF-α 14 days after treatment. However, IL-1β was significantly reduced just 24 h after the BoNT/A intra-TMJ treatment. Based on our results, we can suggest that the intra-TMJ injection of BoNT/A may promote a central effect by reducing the P2X7/CatS/FKN microglia-activated pathway in the trigeminal subnucleus caudalis.

Two antivenoms are available for rattlesnake envenomations in the U. S., Fab (CroFab®, BTG, UK), and F(ab')2 (Anavip®, Bioclon, Mexico) antivenom (AV) with F(ab')2AV released in October 2018. The F(ab')2AV Phase 3 comparative clinical trial demonstrated similar efficacy in treating venom-caused hematologic toxicity, similar rates of Types I and III hypersensitivity reactions, and a lower rate of recurrent hematological effects than FabAV. ⋯ Type III hypersensitivity reactions were similar between F(ab')2AV (36%) and FabAV (25%). The results of our study are consistent with the Phase 3 clinical comparative trial and indicate no significant differences in safety or effectiveness between FabAV and F(ab')2AV. F(ab')2AV offers the advantages of not requiring maintenance doses and may have a lower rate of late hematologic effects in treating rattlesnake envenomations.

In the Brazilian Amazon, snakebites are a significant problem, especially for populations in rural areas, particularly in forests, where victims are a considerable distance from hospital care. Several factors are associated with the severity of the accident, such as the size and age of the snake. This study aims to compare the clinical, epidemiological and laboratory aspects of envenomation to the size of Bothrops atrox snakes. ⋯ The distance of the punctures was related to the size of the snake and the severity of the local envenomation. The observation of the distance between puncture marks when present, which is correlated with the length of the specimen, as well as the estimation of the snake size by the patient, may provide more support for the health professional on the prognosis of envenomation. The use of boots in activities in rural areas and forests could contribute to a lower rate of cases of snakebites, and health education on preventive measures and first aid for populations is fundamental.
