[Intratympanic gentamicin in patients with Ménière's disease: analysis of our protocol].
The aim of this paper is to analyze the peculiarities of a protocol for treatment with intratympanic gentamicin in patients with Ménière's disease. ⋯ Ending weekly injections of gentamicin for the treatment of patients with Ménière's disease when signs of vestibular hypofunction appear warrants a control of vertigo similar to that obtained in other series.

Case Reports[Toxicity of topical administration of lidocaine].
It is frequently described in international literature the possibility of toxicity by local anesthesics. The lidocaine is one of them. ⋯ We want to describe a case of toxicity by topical administration of lidocaine (Xylocain) which caused neurological disease with convulsions, and cardiological disease with ventricular fibrillation, in a patient who came for a thyroplasty. Patient's evolution was satisfactory.

[Intranasal approaches for pituitary area tumors. Our experience].
Transsphenoidal approach is the most commonly employed surgical technique for the resection of pituitary tumors. We present our experience in the transnasal-transsphenoidal approach in 30 patients undergoing pituitary surgery from 1998 to 2002. Ten patients underwent surgery via a sublabial-transeptal-transsphenoidal approach, 15 patients via a transeptal-transsphenoidal approach and 5 patients via a pure endoscopic transnasal-transsphenoidal. ⋯ We conclude that the pure endoscopic transnasal-transsphenoidal approach guided by a navigator-system is safest and more effective approach. We reserved the transeptal-transsphenoidal approach for cases with obstructive septal deflection and poor pneumatized sphenoid sinuses. We do not recommend the use of the sublabial-trans-sphenoidal approach.

[Iatrogenic tracheal stenosis following endotracheal intubation: a study of 20 clinical cases].
The authors report a case review series of 20 patients with tracheal stenosis after prolonged oral or nasotracheal intubation who underwent surgical treatment between 1995 and 1999. Seventeen were male (85%) and three (15%) were female. The age varied between 9 and 68 years (32.8 +/- 14.9 years). ⋯ All patients were treated surgically with an external approach for reconstruction after stenosis ablation. Follow up was at three and six months after surgery there were: 16 cases with (80%) good results, two (10%) with satisfactory results and two unsatisfactory (10%). We found a statistically significant correlation (Spearman correlation) of 5% level between the period of tracheal intubation and patient's age (R = 0.478, p = 0.033).

Case Reports[Spontaneous cervical emphysema and pneumomediastinum with voice disorders].
Spontaneous cervical emphysema and pneumomediastinum, occurring in the absence of previous disorders or provocating factors, is very rare. The predominant symptoms are retroesternal pain, dyspnea, dysphagia and neck pain. ⋯ The evolution is generally good and conservative therapy leads to recovery in most patients. We present an unusual case of pneumomediastinum, cervical and retropharyngeal emphysema that spread to cavum; the suspected symptom was voice alteration as reported by the parents.
