Multicenter StudyClinical performance of the new cardiac markers troponin T and CK-MB on the Elecsys 2010. A multicentre evaluation.
Elecsys assays for the cardiac markers Troponin T (cTnT) and CK-MB have been evaluated in an international multicenter study on the random access analyzer Elecsys 2010 to characterize their clinical performance and their comparability with respective established routine methods. In method comparison studies of Elecsys Troponin T (TnT) with Enzymun-Test TnT, good correlations (r > or = 0.95) and a high degree of correspondence (slopes in 4 laboratories between 0.95 and 1.05) were found. The method comparison studies of Elecsys CK-MB with various CK-MB routine methods lead to good correlations but some systematic deviation in the slopes due to varying standardization. ⋯ The slightly improved sensitivity of Elecsys TnT in the lower range even allows the recognition of pathological increase at cTnT concentrations below 0.1 milligrams/l in special situations with sufficient additional clinical information. Summarizing, provide the two cardiac markers on the Elecsys 2010 at least equivalent or even superior diagnostic information in various clinical situations of cardiac disease compared with routine methods. The short turn-around time and reliable performance qualify the Elecsys assays as new methods of choice for routine and emergency use.

[A protocol for the assessment of patients with coma of unknown origin].
Evaluation of the applicability of a protocol designed for the quick and efficient management of patients with coma of unknown origin (CUO). ⋯ By using our protocol, 98% of the causes of CUO were clarified. The main causes of coma were of neurological, toxicological and metabolic origin. CCT and the blood analysis were the most important procedures leading to a final diagnosis.

Since January 1997 the first insulin analogue, lispro has been approved for clinical use in Austria. Insulin lispro has a more rapid onset of action than regular insulin and can therefore be injected immediately before the meal, providing increased therapeutic flexibility for the patients. In many studies a slight decrease in frequency of hypoglycaemic attacks was reported. ⋯ No data have been published as yet on the use of insulin lispro in children under 10 years of age and during pregnancy. Potential advantages and disadvantages of insulin lispro therapy, as well as individual factors pertaining to the patient must be taken into consideration on planing treatment with this new insulin analogue. Patients should be well informed about the different action profile of insulin lispro, especially with regard to exercise.

Historical Article[Involuntary sterilization of mentally handicapped patients].
Medical discourse on "racial hygiene" and the law on the prevention of hereditary disease in progeny passed in 1933 in Nazi Germany presents us the historical background to the topic. By analyzing six case histories from "hereditary health" court decisions during the years 1942-45, we illuminate how the law of 1933 was applied in Austria. ⋯ There are similarities between the past and present decisions regarding carrying out sterilization of mentally handicapped persons, but the justification given for doing so has changed. The argument of today "protection of the individual" supplants "racial hygiene".
