Randomized Controlled Trial Clinical TrialProspective study on the use of bolus intrathecal baclofen for spastic hypertonia due to acquired brain injury.
To determine if the intrathecal delivery of baclofen will decrease spastic hypertonia caused by brain injury. ⋯ Intrathecal injection of baclofen is capable of reducing the spastic hypertonia associated with brain injury.

Comparative StudyLaryngotracheal stenosis after intubation or tracheostomy in patients with neurological disease.
This retrospective study evaluated the incidence of airway complications in neurological patients following translaryngeal intubation, tracheostomy, or both. ⋯ Fewer complications are associated with transtracheal intubation than with tracheostomy. The data suggest that longer periods of intubation be used when attempting ventilator weaning before restoring to tracheostomy if weaning fails.

Case ReportsBotulinum toxin for spasticity and athetosis in children with cerebral palsy.
Botulinum toxin A has been used therapeutically in humans for a variety of conditions since 1980. Over the past few years, it has been used more frequently for spasticity management. We describe the use of botulinum toxin for spasticity or athetosis management in three children with cerebral palsy. ⋯ In this patient, the injections were combined with other forms of treatment. In this patient spasticity was decreased and function was increased temporarily. The use of botulinum toxin in general is discussed and related to these three cases.

Case ReportsPelvic insufficiency fractures after irradiation: diagnosis, management, and rehabilitation.
Insufficiency fractures of the pelvis are commonly overlooked as causes of severe hip and low back pain. Predisposing factors include postmenopausal osteoporosis, corticosteroids, and local irradiation. Differential diagnosis includes metastatic disease to bone. ⋯ CT-guided bone biopsy was consistent with radiation osteonecrosis. After diagnosis and continued therapy, the patient progressed to ambulation with moderate discomfort. Failure to diagnose insufficiency fractures could lead to further pelvic irradiation, compromising already weakened bones and causing prolonged disability.

Comparative StudyBack pain and peripheral joint pain in an industrial setting.
To develop a self-reported low back pain (LBP) questionnaire and assess its usefulness in (1) describing the incidence of LBP in an industrial setting, compared to medical records and workers' compensation statistics; and (2) targeting specific work sites where the levels and patterns of pain suggest a need for specific intervention. ⋯ This questionnaire appears to be potentially useful in (1) identifying workers who are symptomatic at an early stage of their LBP problem; and (2) identifying pain patterns in different areas of the plant.
