Depression affects millions globally and often coexists with cognitive deficits. This study explored the potential of probiotics in enhancing cognition and ameliorating depressive symptoms in major depressive disorder patients. ⋯ Probiotics may enhance cognitive domains and mitigate depressive symptoms, emphasizing the gut-brain axis role. However, methodological variations and brief intervention durations call for more standardized, extensive research.

Ipsilateral femoral neck, intertrochanteric and acetabular fractures with posterior dislocation of the hip: A case report and literature review.
Traumatic hip dislocation with ipsilateral femoral neck fracture, intertrochanteric fracture and acetabular fracture is a rare and serious injury that occurs mostly in young patients and after high-energy trauma. The treatment of these injuries is a major challenge for orthopedic surgeons; it can have devastating consequences if not treated properly, including avascular necrosis of the femoral head and traumatic osteoarthritis of the hip. In previous case reports only internal fixation of femoral neck fracture was performed without revascularisation and there was a lack of long-term follow-up results. In this report, we fixed the fracture revascularised the patient, and obtained good prognostic results at up to 20 months of follow-up. ⋯ Traumatic hip dislocation with ipsilateral femoral neck fracture, intertrochanteric fracture and acetabular fracture is a rare and serious injury, and surgical intervention is often needed. Because of the high risk of femoral head necrosis in such complex injuries, it is very important to protect/restore the blood supply of the femoral head during surgery. Therefore, in younger patients, we believe that the use of a quadratus femoris bone flap graft and PFLCP is an acceptable treatment for this severe injury. We discuss the management of this rare case and review the current literature to provide the best evidence-based recommendations for this injury pattern. We conclude that for patients with complex ipsilateral proximal femoral and acetabular fractures combined with hip dislocation, the key to surgical treatment, in addition to anatomic reduction and good fixation, is the primary reconstruction of the femoral head blood supply.

Endothelial dysfunction has been implicated in the pathogenesis of obstructive sleep apnea-hypopnea syndrome (OSAHS). Previous research has linked endothelial dysfunction to the vascular pathology marker endothelial cell-specific molecule-1 (endocan). This meta-analysis compared endocan concentrations among adult patients with OSAS and controls, and ascertained whether any differences exist. This study aimed to highlight the association between high endocan levels and OSAHS. ⋯ Results of this meta-analysis provide further evidence supporting elevated endocan levels in adults with OSAHS. Serum endocan levels were correlated with various PSG indices and may be associated with OSAHS.

Joubert syndrome is a rare disease of genetic origin with autosomal recessive inheritance and extreme genetic heterogeneity with more than 40 causative genes. Joubert syndrome 7 is caused by mutations in the RPGRIP1L gene. ⋯ This is the first genetically confirmed case of JS in Colombia, the first carrier of biallelic RPGRIP1L gene mutations with hip dislocation and incomplete glottic closure and the first report of the novel c.3545del likely pathogenic variant causing JS.

A persistent primitive trigeminal artery (PPTA) is a rare embryonic cerebrovascular anomaly. Hemifacial spasm (HFS) refers to involuntary contractions of facial muscles caused by the compression of blood vessels against the root exit zone of the facial nerve. There have been no reported cases of PPTA causing neurovascular contact and HFS. Microvascular decompression surgery effectively treats HFS, but operating on strong PPTA vessels poses challenges. We aim to introduce a more efficient approach for overcomes these difficulties and facilitates surgery. ⋯ In cases where the vessel causing HFS is particularly strong and thick, the proximal Teflon transposition with interposition technique for transposition may be advantageous. This method could simplify and enhance the efficacy of microvascular decompression, without compromising the quality of surgical outcomes.
