Due to the lack of a prior comprehensive review and meta-analysis, the relationship between monocyte count and thrombus load in ST-elevation myocardial infarction (STEMI) patients undergoing percutaneous coronary intervention (PCI) has not been adequately established. ⋯ STEMI patients with a high thrombus burden show an increased monocyte count. A high monocyte count upon admission is a major indication of increased intracoronary thrombus burden in STEMI patients with PCI procedures.

This study aimed to assess the test-retest reliability and validity of the Arabic version of the Pittsburgh Sleep Quality Index (A-PSQI), a 19-item sleep evaluation tool, in a population of medical students and interns in Saudi Arabia. Following a 16-person pilot study, 202 participants completed 2 A-PSQI questionnaires with a 2-week test-retest interval to avoid a carryover effect. Statistical analysis using RStudio included Cronbach alpha, Spearman rank correlation, and the intraclass correlation coefficient (ICC). ⋯ This pioneering study is the first test-retest reliability investigation of the A-PSQI, addressing a critical gap in sleep assessment tools tailored to clinical practices in Arabic-speaking communities. It follows the pathway set by previous studies and serves to validate the A-PSQI in this demographic, providing a vital foundation for more catered sleep medicine in the region, targeting better reach to overlooked monolinguals. These findings advance our understanding of sleep quality among medical trainees and contribute to the development of culturally relevant sleep assessment tools.

Fetal intestinal volvulus, a rare and severe disorder, poses significant diagnostic challenges prenatally and can lead to intrauterine death or adverse neonatal outcomes if untreated in a timely manner. This study reports a case of fetal intestinal volvulus confirmed postoperatively, providing insights into its clinical manifestations, diagnostic methods, and treatment outcomes, thereby enhancing understanding of this rare condition. ⋯ Fetal intestinal volvulus requires multidisciplinary collaboration for diagnosis and treatment. Early recognition, prompt delivery, and urgent surgical intervention are crucial for optimizing neonatal outcomes. By sharing this case, we hope to enhance the diagnostic and therapeutic capabilities of healthcare providers in managing this rare but severe condition.

Ahmed glaucoma valve implantation for congenital ectropion uveae with glaucoma: A case report.
Congenital ectropion uveae (CEU) is a rare, nonprogressive anomaly characterized by the proliferation of the iris pigment epithelium on the anterior surface of the iris, often associated with glaucoma. Due to its rarity and complexity, standardized glaucoma surgical management is limited. To our knowledge, the application of glaucoma drainage devices in CEU is rarely documented. Here, we report a case of Ahmed glaucoma valve (AGV) implantation for unilateral CEU associated with glaucoma. ⋯ Although CEU is rare, ophthalmologists should remain vigilant to avoid missed diagnoses due to its high association with glaucoma. AGV implantation with mitomycin C may be considered an effective surgical management for adult patients with late-onset glaucoma secondary to CEU.

Aortoesophageal fistula (AEF) is an exceedingly rare yet critically life-threatening condition, with mortality rates nearing 100% if not addressed promptly. AEF often develops in the context of thoracic aortic aneurysms, esophageal malignancies, or as a complication of foreign body ingestion and prior thoracic aortic surgeries. This study reports an exceptionally severe and clinically rare case of AEF associated with a pseudaneurysm induced by esophageal stenting. By disseminating this case, we aim to heighten awareness of AEF and emphasize the necessity for meticulous decision-making during esophageal stent placement, along with the importance of vigilant postoperative monitoring to enable early intervention. ⋯ AEF is a rare cause of upper gastrointestinal bleeding. For patients suspected of AEF, it is imperative to conduct prompt and thorough computed tomography angiography while initiating an emergency surgical alert. The proximity of esophageal stents to the aorta may significantly elevate the risk of AEF; thus, a comprehensive risk assessment should precede stent placement in cases involving tumors adjacent to the aorta. Furthermore, postoperative surveillance is crucial to monitor potential aortic invasion by the tumor or the development of an aneurysm near the esophagus, facilitating timely intervention.
