Prediction of Cyclic Fatigue Life of Nickel-Titanium Rotary Files by Virtual Modeling and Finite Elements Analysis.
The finite element method (FEM) has been proposed as a method to analyze stress distribution in nickel-titanium (NiTi) rotary instruments but has not been assessed as a method of predicting the number of cycles to failure (NCF). The objective of this study was to predict NCF and failure location of NiTi rotary instruments by FEM virtual simulation of an experimental nonstatic fatigue test. ⋯ File NCF and failure location may be predicted by FEA. Virtual design, testing, and analysis of file geometries could save considerable time and resources during instrument development.

Randomized Controlled TrialEfficacy of a Buffered 4% Lidocaine Formulation for Incision and Drainage: A Prospective, Randomized, Double-blind Study.
Incision and drainage of symptomatic emergency patients with facial swelling is painful even after local anesthetics are administered. The purpose of this prospective, randomized, double-blind study was to compare the pain of infiltration and the pain of an incision and drainage procedure of a buffered versus a nonbuffered 4% lidocaine formulation in symptomatic emergency patients presenting with a diagnosis of pulpal necrosis, associated periapical area, and an acute clinical swelling. ⋯ Buffering a 4% lidocaine formulation did not significantly decrease the pain of infiltrations or significantly decrease the pain of the incision and drainage procedure when compared with a nonbuffered 4% lidocaine formulation in symptomatic patients with a diagnosis of pulpal necrosis and associated acute swelling.

Randomized Controlled TrialEvaluation of Root Canal Debridement of Human Molars Using the GentleWave System.
Studies using conventional endodontic protocols show insufficient cleaning of root canal systems, often resulting in persistent infection and treatment failure. The GentleWave System (GWS; Sonendo, Inc, Laguna Hills, CA) has been shown to result in a higher tissue dissolution rate in a study using bovine muscle. The purpose of this study was to compare the debridement efficacy of the GWS with a traditional method for cleaning root canals. ⋯ The GWS showed a significantly greater cleaning capacity and reduction in residual debris within the mesiobuccal and mesiolingual canals of mandibular molars and the mesiobuccal canals of maxillary molars than those cleaned conventionally.

Diagnostic Accuracy of Quantitative Sensory Testing to Discriminate Inflammatory Toothache and Intraoral Neuropathic Pain.
A differential diagnosis between inflammatory toothache (IT) and intraoral neuropathic pain is challenging. The aim of this diagnostic study was to quantify somatosensory function of subjects with IT (acute pulpitis) and atypical odontalgia (AO, intraoral neuropathic pain) and healthy volunteers and to quantify how accurately quantitative sensory testing (QST) discriminates an IT or AO diagnosis. ⋯ The proposed QST methods may aid in the differential diagnosis between IT and AO with strong accuracy and may be used as complementary diagnostic tests.

Accuracy of Cone-beam Computed Tomography in the Detection of a Second Mesiobuccal Root Canal in Endodontically Treated Teeth: An Ex Vivo Study.
There is a high incidence of a second mesiobuccal root canal (MB2) in maxillary molars. The study aim is to assess the accuracy of cone-beam computed tomographic (CBCT) imaging in detecting a second mesiobuccal root canal in endodontically treated maxillary molars. ⋯ CBCT imaging is a suitable diagnostic technique for detecting a missed MB2 in endodontically treated teeth.
