Descriptive anatomy. OBJECTIVE.: To describe the anatomy associated with the extensive transmuscular paraspinal approach required to perform multiple intercosto-lumbar neurotizations. ⋯ Multiple lumbar roots neurotizations with lower intercostal nerves already have been proposed by other authors. In this strategy, the use of the spinal cord and intercostal nerves above the spinal cord lesion avoids the axonal regrowth required via the injured central nervous system. Rerouting intercostals nerves down to the lumbar roots at their exit from the intervertebral foraminae is less invasive that the same procedure performed down to the vertebral canal at the level of the cauda equina as we used in previous protocols. Our anatomic study confirms the advantage of the paraspinal sacrospinalis splitting approach in multiple intercosto-lumbar neurotizations. The approach is quick and easy and allows a good exposure of the nerve roots at the thoracic and lumbar levels. The L2, L3, and L4 roots could be satisfactorily neurotized with this procedure.

The outcomes of scoliosis surgery in patients with syringomyelia.
A retrospective review of a consecutive case series. ⋯ The lack of thoracic hypokyphosis seen in idiopathic-like curves is a strong indicator of a possible underlying syrinx. Neurosurgical treatment of the syrinx did not improve the scoliosis. Caution should be exercised when choosing fusion levels, and arthrodesis should be planned with the underlying pathology in mind. Scoliosis surgery has proven to be safe in patients with treated syrinx when spinal cord monitoring or wake-up tests are used.

Multicenter Study Comparative Study
Pediatric spinal injury type and severity are age and mechanism dependent.
Retrospective case review. ⋯ This study has shown that the pattern of spinal injury in children is related to age and also the mechanism of injury. While traffic-related incidents are a leading cause of injury across all age groups, emphasis on fall prevention is needed for younger children. Older children, particularly boys, are sustaining spinal trauma in sporting and recreational activities.

Review Comparative Study
Anterior cervical discectomy and fusion associated complications.
Retrospective review study with literature review. ⋯ Meticulous knowledge of the ACDF-associated complications allows for their proper management. Postoperative dysphagia, hematoma, and recurrent laryngeal nerve palsy were the most common complications in our series. Management of complications was successful in the vast majority of our cases.

Spinal cord morphology and dynamics during cervical laminoplasty: evaluation with intraoperative sonography.
An intraoperative sonographic study for evaluating spinal cord decompression status was conducted. ⋯ Intraoperative sonography during laminoplasty appears to be very useful for evaluating spinal cord decompression status. Our original classification system based on restoration patterns of the space ventral to the spinal cord is considered to be practical for predicting neurologic improvement in cervical compressive myelopathy.
