ReviewSuccessful Treatment of a Patient with Lung Adenocarcinoma Harboring Compound EGFR Gene Mutations, G719X and S768I, with Afatinib.
Mutations in the gene encoding epidermal growth factor receptor (EGFR) are the most frequent driver mutations in lung adenocarcinoma in Japan. Exon 19 deletion and L858R mutation in exon 21 are the most common EGFR mutations. Uncommon mutations, such as G719X, S768I, and L861Q, and compound mutations, combinations of 2 common or uncommon mutations, have also been reported. ⋯ The cancer progressed within 2 months of initial chemoradiotherapy. Treatment with afatinib (40 mg/day) produced a partial response, which was maintained for 17 months with continued treatment. A literature review revealed that lung cancer with G719X/S768I compound mutation exhibited good response to EGFR-TKIs, even better than that of lung cancers with single uncommon mutations.

Case ReportsRectal Neuroendocrine Tumor with Synchronous Pancreatic Metastasis: A Case Report.
Gastrointestinal neuroendocrine tumors (GI-NETs) often show hematogenous metastasis, with the liver being the most common metastatic site; however, metastasis to the pancreas is rare. ⋯ More case reports on metastatic NETs are needed to arrive at a consensus for a standardized treatment regimen.

Case ReportsAnesthetic Management in Tracheal Dilatation for Severe Tracheal Stenosis.
We report the anesthetic management of a 65-year-old woman with recurrent, severe tracheal stenosis who underwent tracheal dilatation. She had visited the Department of Respiratory Medicine at our hospital for respiratory distress approximately 20 years ago, and had undergone laser ablation under local anesthesia. Because of recurrence and aggravation of respiratory distress, she now presented at the Department of Thoracic Surgery, and was scheduled for surgery. ⋯ After restoration of spontaneous respiration and consciousness, the patient was extubated in the operating room and returned to the intensive care unit. In anesthetic management of patients with tracheal stenosis, treatment of hypoxia is important. In this case, we collaborated with the attending physician, clinical engineers, and operating room nurses throughout, and consequently, were able to perform the operation safely.

Case ReportsExpandable Metallic Stent for Endobronchial Metastasis from Colorectal Cancer: Reports of 2 Cases.
In recent years, there have been many reports about the efficacy of stenting for central bronchial stenosis. When central bronchial stenosis is due to metastasis of a malignant tumor to the trachea and/or bronchi (endobronchial metastasis: EM), it is classified as "narrow EM" and "broad EM." [1] We managed two patients in whom bilateral stent placement was required for narrow and broad EM arising from colorectal cancer. Case 1: In September 2011, a 66-year-old man underwent low anterior resection for advanced colorectal cancer associated with unresectable liver metastasis. ⋯ Accordingly, stent-in-stent placement was performed and her respiratory symptoms improved. However, superior vena cava syndrome occurred 1 month later and the patient died. We consider that placing an EMS is effective in patients with tracheal stenosis due to EM that is judged to be an oncological emergency.

Uterine rupture at 26 weeks of pregnancy following laparoscopic salpingectomy with resection of the interstitial portion: a case report.
Uterine rupture in pregnancy can occur in patients with a history of uterine surgery such as myomectomy and Cesarean section. Here, we report a case of spontaneous uterine rupture that occurred in the early third trimester in a pregnant woman who had previously undergone laparoscopic removal of the right fallopian tube and interstitial portion for treatment of interstitial pregnancy. The patient presented with sudden onset of abdominal pain at 26 weeks of gestation. ⋯ In emergency laparotomy, the fetus was delivered by Cesarean section, the placenta and membranes were removed, and the uterus was preserved with closure of the rupture and wound. This case highlights the importance of close follow-up of a pregnant patient who has previously had a uterine incision. The case also raises the question of whether the prevalence of uterine rupture may increase as more patients are treated with laparoscopic surgery of the uterus.
