Case Reports[Bullous pemphigoid and amyotrophic lateral sclerosis].
Bullous pemphigoid is the most frequent blistering disease and has been found associated to several neurological diseases, including amyotrophic lateral sclerosis. ⋯ The association does not seem to occur by chance and we suggest an autoimmune pathogenetic mechanism consisting in a crossed reaction between bullous pemphigoid antigen 1 and the protein dystonin, which is involved in the organisation/integrity of the neuronal cytoskeleton.

[Metoclopramide as a painkiller for intense migraine headache in emergency departments].
Migraine headache is an uncommon but disabling clinical entity when patients go to an urgency room with a severe migraine attack and digestive symptoms. Intravenous non-steroidal anti-inflammatory drugs, paracetamol and triptans represent the first line drugs in the migraine therapy in emergency departments elsewhere however the low efficacy and the limitations of the oral way for vomiting determine a difficult situations in some migraine patients. Metoclopramide is an anti-emetic drug which indication in migraine for vomiting and as an adjunctive drug to other analgesic has been reported elsewhere but the analgesic features of this drug in monotherapy has not been largely study in the medical literature. ⋯ The patients showed a important improvement after 10 mg of intravenous metoclopramide exclusively with no other analgesic drug.

To provide a summary of the different experimental models of cerebral ischemia designed both under in vivo and in vitro conditions. A clear and concise description of the specific types of brain lesion reproduced by each model is given together with the most frequent technical troubles associated. ⋯ The use of experimental models of cerebral ischemia constitutes the most suitable tool to investigate the physiopathology of this type of injury. However their simplicity prevents an exact reproduction of the cerebral damage observed in clinical settings. This could be the main reason for the discrepancies observed between the therapeutic effect in the experimental and clinical studies.

[Clinical usefulness of transcranial Doppler ultrasound imaging in the diagnosis of cerebral vasospasm in subarachnoid haemorrhage. A validation study].
During the clinical course of aneurysmal subarachnoid haemorrhage, cerebral vasospasm is one of the most dreaded medical complications. AIM. To determine the usefulness of transcranial Doppler (TCD) ultrasound imaging in diagnosing vasospasm in patients with subarachnoid haemorrhage (SAH) who were attended in a Stroke Unit. ⋯ Monitoring with TCD proved to be useful for diagnosing cerebral vasospasm in patients with a good initial clinical status.
