Breakthrough adverse reactions to low-osmolar contrast media after steroid premedication.
The purpose of this study was to review the nature of adverse reactions, or "breakthrough reactions," experienced by patients who received steroid premedication and low-osmolar contrast media. We compared the demographics of patients having these breakthrough reactions with those of patients who did not develop these reactions. ⋯ Breakthrough reactions occur in a substantial number of patients despite premedication with steroids and use of low-osmolar contrast agents. Typically the breakthrough reaction is of similar severity to the patient's initial reaction. Severe or life-threatening reactions are seen in 24% of patients.

MR imaging of intrahepatic cholangiocarcinoma with pathologic correlation.
The objective of this study was to determine the MR imaging features of intrahepatic cholangiocarcinoma. ⋯ The combination of the signal intensity on T2-weighted images and the enhancement pattern on contrast-enhanced T1-weighted images showed good correlation with the pathologic findings of cholangiocarcinoma. The occurrence of a central hypointense area on T2-weighted images is not pathognomonic; however, this finding, which reflects severe fibrosis, appears to be a characteristic marker of intrahepatic cholangiocarcinoma. The presence of intrahepatic bile duct dilatation may indicate cholangiocarcinoma, although it is difficult to differentiate cholangiocarcinoma from hepatic colorectal metastasis.
